Verb tense disagreement during materia melding

After a failed materia meld attempt, the following text appears:

"[First] [Last] is unable to attach the materia to the [gear]. The [materia] was lost."

If I'm not mistaken, isn't this grammatically incorrect? Instead, I believe it should read either "[First] [Last] was unable to attach the materia to the [gear]. The [materia] was lost."

OR

"[First] [Last] is unable to attach the materia to the [gear]. The [materia] is lost."

best thing to do is run all that through a Grammar check program in a program like Microsoft office (i know not the best) and see if it says if it is a grammatical error or not.
