How could I test or find out if it is indeed hardware issues? I've used furmark to benchmark and see how it performed and it did very well.
Does it happen in any other graphically intensive games?
What temps are you getting while playing?
I am running a 680GTX and I have that EXACT issue. The way I fixed it was by updating to 327.23, in Nvidia control panel adding Final Fantasy XIV Realm Reborn to 3D settings under the "Program Settings" tab, forcing vertical sync on and making power management mode "Prefer maximum performance". I then had to make sure my game was running in Full Screen mode. Boarderless windowed did not seem to get any of the nvidia control panel manual settings implemented.
Hope this fix helps you! My system seems to be stable with the above mentioned steps.
I was previously having issues with driver resets using 314.22 on my 670.
326.80 fixed those errors, and it's also been fine since I went to 327.23

Generally, you are looking at issues centered around heat and/or power if you are having issues with GPU/RAM timings. Typically, you run into this when overclocking--if you are running into problems requiring you to reduce it from stock clocks, then there is something physically wrong with your card or it's environment. Double check things like power supply, ventilation, fan/heatsink, etc. to make sure those main elements are stable and such.
I've had the 327.23 drivers for a couple days now. Absolutely 0 crashes with my GTX 670. =)
