On your initial mob or the group as a whole. If its the group and you have an aoe you need to flash. I normally will shield lob>flash>FB>flash>savage>ROH. At that point mob one is normally dead or cloase and I just cycle the steps in my combo through mobs remaining to keep threat.

In some groups I catch myself doing that exact same rotation Bwalker. That extra flash can be needed, particularly if AOE is going down.
