Group based leveling, challenging, fun, rewarding, new, different, lots of outside world content, some instanced content(not a lot), story based progression, no gear wipes/resets, no gear tiers, no level cap increase after the initial increases(say, it gets increased to 99 then stays there for 5+ years), good strong rewarding crafts that make the best gear in certain slots, even at the level cap, level cap bosses that drop mats to craft into gear.

My perfect MMO, huh? Easy said!
It has to have a beautiful lore, that you can use to do fantastic quests and instances.
A PvP arena, where you can practice playing with other players/parties before going into a tournament fightibg players/groups.
Single-player friendly with a lot of party content, where you can solo to cap, but doing it faster and funnier in a group.
Achievements are nice. I like to see what I've accomplished during my adventures in this MMO.
It has to be challenging! Having the powerful stuff at the beginning lets you see high damages, but is Lame! I want my character to be a Total Sh*t, and then become a savior.
Things like economy, weapons, stats, etc. can be like any old MMO, like WoW or Ragnarok Online, but this is my perfect virtual world.

Take the Everquest out of Final Fantasy XIV, add in some Demon's Souls, Minecraft, and Assassin's Creed. Done.

My perfect MMO would have reasonable graphics, interesting storyline which immerses me and feels like I'm part of it. I also want plenty of lore and content, be able to explore and encounter surprises out of the ordinary, and feel part of a community which likes the game they are playing.
a lot of unlockable content. It's what keeps you around for a long time. If you have to do X, Y and Z to get to zone X2 then you can't explore everything in the beginning and exhaust the game without really playing it.
Or to unlock skills, spells, story, quests, classes, jobs, professions, recipes, zones, items (and upgrade them).
Player housing. Would be nice if it was somewhere you can actually see... Like a zone just for player housing. So you can actually see player houses (with limits to sizes etc otherwise in 5years everyone will have a castle and you have zone with 15000 castles :P)
Lots of variety in gear. Being able to have complete control over the looks of your armor. I would like to choose the color of my armor. It would be great to be able to combine gear to have the looks of something but the stats of another.
I like beast tribes attacking player cities and us having to defend it. If we fail then we would lose some benefits (like bonuses, services, tax increase etc)
PvP for sure even if limited to just certain zones/battle grounds/ instances etc
An achievement system and a way to measure how many hours you have spent in the game (lol)
a lot of animations when i cast the spells (like my hand movements etc) lots of flashy effects (from spells and weapons skills)
many many many variety of zones. They can be smaller... Lots of different themes.
Snow and arctic, forests (night/day/elfy big trees small trees... can be a lot of different forests), jungle, lakey, island, deserts, saharas, undewater, dark and scary or dark and mystical. Beast tribe home zones with lots of nasty aggro monsters.
Different aggro type monsters! Low HP, smell, sound, sight, weakened etc
Jumping, maybe even flying (in whatever form, dont mean on a dragon or with wings just any type) swimming, climbing, underwater stuff zones and content quests etc
A good game needs to have a good economy. An auction house type is a must (no price history pls.. items r worth what they r now, what someone else paid a month ago...)
interactive items in zones (like I want to pick that stone or stick from the ground and throw it at the mob)
A fun gathering system where you can see what you are gathering.
end game that matters, so not going to increase cap every year. Make a cap, decide set it and keep it for a while.
many different looking mobs in level appropriate zones.
No junk items.. I want all items to be worth something.
great story (did I say that already?) beating many big bad world and my beautiful city threatening a$sho1e bosses
plenty of different travel methods (pirates attacking ships yeeeessssss!!!)
Overall, cool story, lots of cutscenes, lovable characters flashy animations beautiful effects (AND SUMMONS!!!) great graphics LOTS OF VARIETY and more content than I could handle...
edit: ADD RELICS for those really hardcore who just have all the time in the world to play their favorite game. Let them show off their badassness and awesome weapon to us (and we can drool over their weapons which would take ENOURMOUS amount of time, money, friends, resources and gametime to accomplish). Only meant for a very very very selected few...

To be honest, I'd like a decent Mature Rated PVE MMO that has a deep meaningful storyline, great character customization in the vein that you learn things from using skills not leveling classes. IE You go to a magic school and learn the basics of magic. They teach you the basic healing magic spell and every time you use it you get stronger at healing magic. Once you hit a certain level you automatically learn new spells from the healing magic school. etc.
Weapons all act differently and performing a skill on one weapon varies with how its performed on another.
Armor is Varied, but the "Theme" is something like Viking Medieval.
Hell Make it full blown Norse. The players are Einherjar on Midgard Training for Ragnarok and fighting off the forces of Hell as they try and invade and cause Ragnarok to start early.

I have to thank Square-Enix for the amazing job they have done recreating Final Fantasy XIV from Scratch. Especially the inclusion of Missing Genders which we petitioned for in good faith. This was proof to us players that the Developers are truly Sympathetic to our requests and that being honest and vocal can pay off with the amazing characters we have who are Female Roegadyn, Male Miqote, and Female Highlanders. Thank You SE, Thank You Community Team, Thank You Yoshi-P.
I hereby admit to being "really lame". To elaborate further, take the grind out of FFXIV, add in the multiplayer content-focused mission structure of Demon's Souls, and add in the creativity enabling of Minecraft. One reason FF online games aren't as popular as offline FF games is that FF games don't have grinds in them. That's where Demon's Souls comes in. It's the perfect online game. There is no grind, just players playing together, having adventures. It's the best. That's what FF is searching for, but like someone said in a recent thread, they weren't smart enough to figure out how to attain it. Devs, go play Demon's Souls!!
Demon Souls was ALL grind :/ Fun grind, but still a lot of grind.
