Yes, you can change your name. Here is the information on how to do so.

[NA] Regarding the Character Appearance Change of Your Version 1.0 Characters(Sep. 5)

We have determined our course of action to address the situation wherein players were unable the load their character data from the official benchmark when changing the appearance of their FINAL FANTASY XIV Version 1.0 characters. Please see the details below.

[Character Name Change]
If you experienced an issue when trying to change your Version 1.0 character's name upon logging into A Realm Reborn for the first time, please fill out and submit the form below by Sep. 10, 2013, 7:59 a.m. (PDT).

Character name changes will begin from Sep. 11, 2013 in the order in which they were received. After your request has been processed, the Character Name Change Screen will be displayed the next time you log in, and you will be able to change your character’s name.

Please note that once you receive the Character Name Change Screen, you will no longer be able to choose the same name your character currently possesses; you will have to choose a different name. Please be aware of this when you submit your request.

[Character Appearance Change]
Players that experienced issues changing the appearance of their FINAL FANTASY XIV Version 1.0 character when starting A Realm Reborn will receive a consumable in-game item called “Fantasia.” Using the item in game will allow you the opportunity to edit your character's appearance. The Fantasia will be sent via the Moogle Delivery Service in the order in which requests were received.

Please note that because Fantasia is a rare item, a character cannot possess more than one at a time. If you already possess a Fantasia (obtained as a Veteran Reward, for example), please use your existing Fantasia in your inventory before receiving the new one from the Moogle Delivery Service.

I would consider a name change but they are specifically saying it should be used for people that had trouble renaming their character. I would like to get someone from SE to officially say they don't mind if people who didn't explicitly have trouble renaming use the name change application.

The phial really should come with a name change. I really don't want to change my race/gender if I am going to be stuck with a name created from Square Enix's official naming conventions...

I believe if you send in a request, it may be something they can sort out for you.

This one is the do-over for the clusterfrak that happened during OB/EA... everyone will get one after their 30 days of being subbed.

Give it time. I just got mine tonight at 9pm. I am sure you will have one soon if you are legacy! no you don't have to subscribe to anything. They know you are a 1.0 player.
