If only I could get a hold of someone to escalate mine... AND I'm on PS3...I'm a NA / PC user. I submitted my form last Tuesday and today I managed to call in. After about 40 minutes on hold, I talked to a guy named Jack. He verified my account, asked for my ticket number (which was the one I got from submitting the necessary form), and escalated it. Checked my mog station minutes ago and my first service account now has my collector's edition items added to it.
So, NA users.. hold out hope that it is being taken care of! Escalated accounts are getting priority though.

Just got off live support with Squenix. They said for PS3 it could possibly take a couple hours, days, or up to 2weeks.
Been on hold for 3 hours and 137 on queue from start queue # of 304
