The smaller slip that is red and black is a SE members code which you can log in and register the game on your square enix account. This isn't mandatory at all the only code you are concerned about is the 12 digit.

The code from preordering is the preorder code. the CE item code is the same as the CE game code.

Yes when you pre-ordered the game you got a code that gave you early access. However depending on where you purchased from you also got a serial key that gave you access to the CE bonus items.
Like I said I have the CE on PC not PS3 but I know people who pre-ordered on PS3 and got codes. I am sure the PSN code will get you the items but I can't confirm that
As you can see the code I got from pre-order registered the game but the code is the box is for me different as you can see I have two PC versions registered but own only one.
