The scary part is they don't seem to have a plan for how to fix it yet. They've done how many maintenance periods now, 4? Nothing has been fixed and there's still no word on any new maintenance to fix everything.

well, in all fairness they are just getting into work on Monday now. they obviously did not leave their 'best' over the weekend to handle the launch.
I "think" the Japanese take defacing hard, kind of like the French (no offence, I work with the French almost every day!). you can't have the conversation "we learn from our mistakes" like you do in NA (and UK that I know of). Its a really big insult to even bring up someone's failure;; I work with a very diverse group (but no one from Japan) and have seen this; its hard to 'Americanize" them =)
