C'mon, more often than not, it's a medieval fantasy or heavily inspired by it, it's just not a rule.

Final Fantasies 7, 8, 10, 13, and 15 would like to have a word. Hell even in 12 I would argue it is more steam punk level than Medieval fantasy. So that would actually make the pure/heavily inspired Medieval Fantasy style Final Fantasies the exception.
