The area is meant to represent one portion of Garlemald and its outskirts, but actually having to spend an hour walking from outlying villages to the city would get very boring very very fast, so it's all shrunk down for gameplay convenience.
Not only that, but remember that across all of Garlemald, we visit two different train stations outside of it, and travel through a third to get into the Tower of Babil. Even if we assume that train journey to be essentially one stop, the fact the trip necessitated a train line be made in the first place suggests there is a substantial size to the city.

...hell, there's most likely two train lines, given that the one we take into Babil is likely for freight while Tertium is clearly for passenger trains.
