All of us sitting here wondering what could the ESRB possibly be referring to with the partially nudity and Sexual themes.
Note: a certain cat being pulled from a certain place twice, doesn't count because said cat was always covered in blinding light.

It's probably to do with some of the Sin Eater designs, I guess. Though FFXIV seems to have moved away from suggestive content over time. ARR was full of innuendo and suggestive themes but there's been significantly less of it ever since HW.
Yet here in the UK the game has the same rating as Shadow Hearts did and that game had a lot of suggestive content in it, without really going too far overboard.

A modern FF that isn't T would be "off brand" and intentionally going to M is not done lightly. Changing a subscription game from T to M would almost certainly cause some major backlash.

Well, Final Fantasy Type-0 is rated MYeah, that's pretty minor. But you know that a full rating change would be a problem. ESRB ratings and other ratings have become like part of the brand of certain series. A modern FF that isn't T would be "off brand" and intentionally going to M is not done lightly. Changing a subscription game from T to M would almost certainly cause some major backlash.
