Typically, people tend to kick players that want to start fights or drama (aside from the usual AFK/offline for too long dismissals). I only kick for people that are deliberately causing trouble, or for players that are leeching (e.g., back before 24-mans auto-zoned you in and some players would back out of a boss room to avoid participating).

The last kick I did was actually against an AST in Orbonne. I was on PLD, and I wasn't main-tanking any of the bosses; but they kept putting Boles and Regens on me. I told them they didn't need to do that since I was OT, and thy got so mad at me for "telling them how to play" that they refused to heal me during TGC and I died to the buster (no Esuna on the debuff and my Clemency didn't go off fast enough). After we wiped to TGC (because they were also refusing to heal most of the party due to arguing with a friend of mine in chat), we kicked them. Went through almost instantly. When you behave like that AST, that's when you should worry about being dismissed - though some groups just grin and bear it. Quite people that do their job and don't interact much are rarely targets for dismissals in my experience.

Or repop instead of waiting for a raise, or plain refuse raises. That's an instant vote-kick for me.

It really depends, apart from the case I just mentioned, you really need to put a lot to trigger a vote-kick with me in regular content. Most of the time, I don't read party / alliance chat if there's nothign wrong so I'm out of the drama. Sometimes some runs take a long time and I just think "it happens" so I won't track whose fault it is.

In a Savage / Extreme primal setting, I've seen some kicks due to underperformance of players (healers mostly but sometimes DPS), and while I agree it's annoying I think it's fair as long as it's justified : we died on a healing/DPS check several, there's a problem. In the end, everyone is losing their time so I'll agree at some point you need to break the team and start again.

Context matters, if someone is given grief for using the wrong fairy (Eos, Selene) and it's a regular duty, that is pathetic on the other persons part however. Once you reach the level of Savage I do change my criteria, unlike other content I can have a reasonable expectation of skill and if you clearly suck and don't know the fight or even worse your rotation (and its not learning) then I will kick people. I am not here to have my time wasted and again context matters, I wouldn't do that in regular instances but once we are talking Savage then its different rules but it goes without saying since its like an unwritten rule you'll be kicked if you haven't even learned your rotation

    I’ve only been kicked once.
Because they didn’t like that I was DPSing as WHM.

Their own words, even. Their loss, I guess.
I also don’t understand how people find themselves kicked so often it’s a notable thing to them.

The irony I like to see is that you were probably doing the same if not more DPS then the actual DPS themselves.
