If you can't take the occasional RNG driven quicksynth fail, then craft it manually. If the already low failure rate were lowered even further, what would the point be? Just tell the game how many you want and *bing* they're done...

I quick synt by 200 stacks or more, the onle way I fail them is if my gear condition reach 0 an I don't notice,
make sure all pcs of gear are in good condition.

I try to repair any time any of my things falls below 100% though, and since I see all of my gear's condition every time I open my character screen to change classes, I usually catch it well before anything breaks!
As others have stated, if you are failing Quick synths you need more craftsmanship.
I personally can not remember the last time I had a QS fail on me that wasn't me being dumb and trying to QS things above my level before 60.
I failed level 2 recipe quick synth on my lvl 15 cook. :<

The only thing that bother me is having HQ quicksynth...
Because my inventory is almost full =X
