The only time I've seen an 'Adoption' system in an MMO was in the original Ragnarok Online - "baby" status player characters.... two player characters 'marry' and gain special abilities, etc, then can register and 'adopt' a third player's character with permission (the 'baby' turns from a normal sized character into a tiny, cute version of the game sprite). If all three players are in a party, then all three gain extra-extra abilities in battle/dungeons (I believe it was xp boost and stat boost? It's been a long time). While the 'baby' characters were adorbs (and stat/lvl-capped >.<), it managed to somehow not be incredibly weird or creepy (I guess less detailed character sprites have that advantage?). I had a baby character there, for shits and giggles with friends, at one point - pretty amusing to watch a tiny baby GFist monk bobble around the parent players, in a mobby dungeon, and then suddenly Guillotine Fist something for a few thousand damage.
I doubt they will implement something like this in FFXIV, the current NPCs are just to flesh out the current 'world' - but if they did, it would probably be more NPC-ish in nature. Either with minion-type mechanics (auto-follow, random roaming movement when player idle, some randomized action, etc) and "dissapear" when dungeons are entered. But then it would beg to ask which parent it would follow if it was one shared between two people.
Or maybe it would wait in the Inn room (if no house or if the 45 day house demo happened) or players private room(s). I wouldn't want to see what would happen if a large FC with a lot of adopter members had 463856943650365943 kiddies in the FC house >.>;. Crowded with moving NPCs on a pre-determined track (as are 'patrolling' city NPCs). And then there's collision detection - moving around objects players put in the houses - or if they did have an environment-collision coded into the AI (somewhat like enemies), players could trollingly trap them with furniture.
Or maybe a pre-req would be a private house (and share-ability of the house with married players) that it would stay in, like the mender/tool NPC, etc....

Honestly, I think I would get too attached to a Mi'qitten and feel bad about not visiting it if I don't go on for a few days. >.> (whereas I had no such attachment to RO "baby" characters).
If they allow us to play as children, I'd switch in a heartbeat. The thought of saving the world as a child sounds very appealing. Atreyu-ish and such. :3

Meh if the parents did go offline for awhile just leave a few lalafells for the kids to eat while offline lol but seriously that would actually be kinda kool having a kid npc running around the house just like in fable but have a limit as to how many parents can have maybe some play outside others inside and the parents just choose what they look like kinda like how they pick what the colours and music would be for bonding ceremony
I'm perfectly fine with the idea of having AI controlled "children" from two players in game. I'm also perfectly fine with having adoptable children in game for either same sex couples, or single players that simply want a child. HOWEVER, I can see major issues with the idea of having children running around in the companion slot. If they implement something like this, it should be in the manner that the children are restricted to being in the interior of parent(s) homes/FC private chambers, until they "grow up" in game, (maybe have a month or so cycle of being children, then young adult, then full adult). Once they are fully grown, they should be able to follow you around on adventures as NPC party members, with similar equipping rules as retainers. Otherwise, we're going to have way too many issues with players doing things that frankly shouldn't be done, if we have children characters running around everywhere.....
For similar reasons, I am STRONGLY against the idea of actual players being able to play as children. The people who would do wrong things with that FAR outnumber the number of people who would simply role play as an actual child.....
