I shall join in on the chorus of posting our own derp moments! Because I too have had my share, especially during the earlier years when I first started. To break it down:
Derpish stuff I did back then and haven't done since: (Yet)
1: During my very first dungeon run ever as an Arcanist, I had Topaz Carbuncle out for the whole dungeon run. (The tank didn't say anything about it) Later on, I came to realize that Topaz, being a tank pet, doesn't do as much damage as Emerald Carbuncle, and failure to realize that a competent Tank player is all that's needed for holding enemy aggro. Since then, I've never summoned tanking pets in dungeons again... unless I see that the player is being a "lolTank" and not holding aggro effectively.
2: Related to 1, I used to always have my pets set to "Sic". This was failure to realize on my part that the pets Ifrit and Titan had stun moves, (and Garuda kinda) which they WOULD use if set to "Sic" and build stun immunity on the enemies.
3: Related to 1 and 2, during my first Ifrit HM fight as a DRG, I kept throwing out leg sweeps and Spineshatter Dives out of habit for extra DPS. I failed to remember that these moves inflict stun, which while not entirely needed for the fight, it DOES make things easier when you can stop "Eruption" from going off during the final phases of Ifrit. I facepalmed myself over not realizing that I made it a bit harder than it should have been.
4: Suggesting that my friend be a Gladiator when we first started the game together, not realizing at the time that A: It's a Tank class and B: He's a very horrible Tank. (All he can usually think about is killing stuff. Little details? He's not very good with little details) When another friend (and now a FC member as well) joined a little later and noticed it too, we both agreed to persuade our "Tank" to never tank a dungeon again. It was hard to get him to stop too, due to his insane love for swords. (The biggest reason why I suggested the class to him in the first place) After agreeing to get him his PLD relic weapon, (because he's that stubborn on collecting everything he can so much as touch in games) he's never tanked again. Good thing too IMO. Guy could hardly hold aggro for the life of him, let alone dodge attacks. He makes for a decent BRD though, as both me and my other friend agreed on. (IMO, BRD is the easiest class/job to pick up and play as anyway)
Derpish moments that I may still have on rare occasions:
1: Any time I activate sprint as a melee DPS or BRD by accident... Double the points of frustration if I did it as a PLD. Triple if I did it as a WAR. (How the hell do I hold aggro now!?)
2: One time, I elusive jumped off the arena in Titan HM as a DRG. Yeah... talking about having a lolDRG moment...
3: Related to 2, I once Shoulder Tackled too soon as a MNK into a Landslide. Didn't die though.
4: Attempting to activate Hallowed Ground and thinking I did (cause I have a macro for it), when in truth it had like... .5 to 1 second left on cooldown, resulting in me NOT using it and taking a massive hit for it. Similar results with huge pulls on rare occasions.
5: Flipping through my hotbar menus too fast or not at all (for whatever reason) and thus pushing the wrong buttons. To explain briefly, I had this moment in WoD:
*During battle against the first boss as I'm playing as WHM*
Brain: "Ok. Adds are coming. They need to die ASAP. We can hit both if we use Holy while keeping our main target cursor on the MT, so that we know when to pop out of CS and heal as quickly as needed. Don't forget to throw out a Medica II in there just in case the adds use that damn AoE attack."
Fingers: "DERP!" *Somehow switches forward a hotbar instead of going back to Hotbar 1*
Me: *Pushes button where I had the spell "Holy" set up on Hotbar 1... but is on Hotbar 5 instead. Cue the following LS message in chat for my hunting LS:*
Chat Log: "BOUNTY MARK IN SIGHT! *Insert player name here* is hiding at *pos*!
Brain: "Wut?"
Said LS got a laugh out of it anyway. "The new S+ hunt I've been hearing about?" one of them asked. Even though we all got a laugh out of it, I was still doing ">_<" at the end of it, lol.
