This made me twitch a little. I'm sorry, but you have to define what is a "good healer". It's obvious that good healer doesn't let people die (accidents happen of course), knows how to manage mp, doesn't overheal to steal hate from tank etc... Then here comes the other part where people will start have different opinions; is a good healer someone who can throw in some dps? IMO doesn't matter as long as you can manage to heal.
As far as commendation goes for me, well... You get one from me if you're a cool kid, have fun and do your job. Last time I gave commendation to someone was a BRD in Snowcloak and he greeted us with "Hello" and sang Foe's to our BLM(was running with FC mates for relic). I didn't get his commendation in the end but did I mind? No, cause I had fun.

Because there are many DoTs in low dungeons that are utterly ineffective regarding the damage they do. Like the poison in Totorak, you are better off ignoring it completely instead of sacrificing GCDs for those.See, I'm leveling a healer now (finally) and I don't get why anyone WOULDN'T do this. Because I'm going crazy on Scholar without this spell for now.
Seeing a DoT on someone is freakin' stressful. Like someone low on health, it's a problem that needs dealing with ASAP. The game is so clear about what you do in your role, that why would you think for a moment you're NOT supposed to fix that?
However, status effects like slow or paralyse should be esunaed asap.
I main sch and war but have all classes 50. My xp is out of ca 1200 commies (I barely run stuff not premade) I made around 100 while leveling dragoon 70 while leveling pally (only 2 classes I leveled after commies were implemented and nin I grinded crazily in 3 days with friends so cannot count). As sch I got majority around 400 commies war I guess 300 the rest is randomly of some other classes... I lately play lots on dps classes because the light grind made me sick of tanking and healing dungeons and I rather go lolwutt??? when I leave a duty with 3 commies.
I think tho its not only a matter of role, there are many reasons to commi someone:
Be it skill (and be it only the 1 nin that doesnt goad the tank or the bard that keeps straight shot up/the drg with heavy thrust), a funny joke someone made, a hint someone gave on rotation or boss and there also is the trading (I do it at times when someone helps us out in progress and noone impressed me)
These are actually the points that make me give em but its up to everyone himself.

Most of my comms go to people who are generally just nice and helpful to the group, especially those who just entered the dungeon first time. There was this one tank who helped our group through Halitali HM as two members of our group were new to it (including me). He was very patient and gave us advice on the boss battles. =3
Anyway, I prefer to give my comms to people who deserve it. I don't waste them on people who bully or belittle other players. Sure they may be probably pro at what they do. But there's no excuse for acting like a jerk to other players who are still learning or trying to do their best. It doesn't matter if you're tank, healer or a DPS, if you are a nice and friendly player, who helps other players, give advice on bosses when needed and just enjoy the game and have fun instead, then you will earn my comms. Anyway, that's just my opinion, you can either agree or disagree. =D I apologise if I ranted too much. I tend to get carried away sometimes. XD
The commendations are given more on my opinion of weather your
A a fun person to be around
B a wipe saver
And lastly C just the only nice person in a party.
Their not really used to measure a persons ability or for that matter competence.

Perhaps your friend is actually a great player and/or friendly.I was talking to a friend and he said that tanking from 1-50 netted him 400+ commendations I am a lvl 50 SCH and have almost unlocked everything and am at 160 commendations... I know I am a good healer, so it has nothing to do with that either. I just don't feel like people respect healers as much in this game as tanks and their job is just as important. .
God knows I rarely give any tank a commendation. (very few good, or friendly tanks around)
Personally I always commend the nicest person in the party. If there isnt one (because noone talked), I will commend a player that I noticed playing well.

Or the even longer time since there are multiple dots and you need 2.5 seconds to clear just one.

I try to give com to whoever I have identified as having been THE one in the group....
Tanks : if they look what is around them and actually play their class rather then be on auto-attack
Healers: if they actually heal rather then DPS without ever bothering healing even if tank is near to death
BRD : a BRD will always get my comm if he uses his abilities to help the group without being asked, I have met some of them, peopel too often forget to say ty to them when they do use those spells
DRG : probably, for me, the hardest class to judge but they do get my comm based on overall attitude
BLM : were they effective or ineffective
SUM: I still wonder when a SUM doesnt use his DoTs to help the group, but someone using their pets and dots and heals when needed are just THE player.
other then that, is comm are often subjective rather then objective. It happened that I gave no com to any as, in my opinion, none deserved it.
Giving a com is subjective. Nice if you get them but doesnt matter much if you dont any, not getting any doesnt make out of you a poor player
On a side note, to OP, the perception you have of your healing skills and what the others have may be totally opposite, jsut because you THINK you are a good healer doesnt mean that you effectively are one.
