I notice the normal shield skills, tank skills etc..

A few oddballs he uses a lot:

Emulate
Accomplice
featherfoot
Decoy
Diversion

Helps a lot.

Other things that might trigger it:

Every single enemy killed (no depopping, including all enemies at end of random tunnels)
Activating the 4 circles in skeleton room in certain order (24 possible combinations)
Never getting hit by either Ahriman at any point (most difficult spot right after Ogre)
Incapping Ogre (Horn?)
Incapping Bartaal (Horn? Arms?)
Killing Bartaal without killing any Skeletons. (off-tank all 8... GLA might be able to do it abusing shield skills)

Some idea's, if any have been done feel free to say and I can strike them out.

Have you tried killing all of a certain mob type? All frogs, all imperials, all drakes etc. ?

oddballs as in you don't know why he uses them or people don't generally use them while tanking?

cause those are all very strong skills.
