I was sort of lazily building spirit bond on some old cnj weapons/multiples (like, lvl 23ish), on my WHM, by using quick, low level guildhests (didn't feel like doing whole runs as I doing laundry/cleaning. I figured, its low lvl hests, what could go wrong. Apparently nothing, for a while - til one run (of many actually) of 'All's Well that Ends Well'.
Got to the 'boss' at the end... squishie tank wasn't holding the ADDs/flashing, everything would switch to me and the DPS weren't killing the ADDs. More ADDs spawn on top of the other set. Tank keeps getting hit hard so I keep healing and pulling ADDs (that DPS still did nothing with). Rinse and repeat. I'm okay with tanking junk but not when I've got the whole fight's ADDs while dodging AoEs and spam healing/esuna myself and a squishie tank.
Long story short - I died.
Not so much a terrible horror story like the others mentioned, but to die in a level 25 hest is just...

Every Ultima (Hard), ever. Except once.
Otherwise, accurate depiction of my experiences, ever:

Castrum Meridianum - The MT decides to go full speedrun after the black eft and without warning pull aaaaaall the mobs from the area up to the pipes, there were so many mobs he had to run around in that small platform so he wouldn't get hit with aoe, results: miraculously only 2 DPS died and me and the other healer managed to swiftcast raise them in time to finish off all those mobs... It was chaos, I barely survived, just AoE on top of AoE, I couldn't understand what was going on screen, all I saw was the health bars going down on the party screen, I had to use lustrate repeatedly on tank for him to survive. After all that I said "that was crazy", he replied "oh? was that too much?"
Titan Hard Mode - Somehow we managed to win with a dragoon that literally ate up EVERY SINGLE AOE, he died twice by being pushed (we did it on the third try) and he didn't say a word through the entire trial.
Haukke Manor HM - A bard was undergeared for this, our dps was low and he had very little HP, through the dungeon he would always be the first to die. We couldn't kill the last boss, she would always do a painful blood rain (almost killing him) because we couldn't kill the last add quick enough, she would one shot him with void fire after the stun and we couldn't kill her before she used her third stun so we slowly wiped everytime.
