There were a very very rare few times where Blizz has done that. Most infamously during the Lizard Squad DDoS debacle. However, they have never actually given any credit for actual maintenance scheduled or Emergency.

Servers crashing wasn't uncommon, but they were rarely down for long periods. I remember most of the time they'd be back up within 15 minutes. Sure the weekly maintenance wasn't great either, but it was rare that things were unplayable for a full 24 hours, let alone 48 hours, but it was those rare cases where they did actually take that long that active subs were credited.
You sure about that? I distinctly remember a few times where they did credit people. It might have only been for certain servers that were particularly finicky, I played on one of the more populated ones.
Either way, as I mentioned every other MMO I've played that had extended downtimes has credited active subscribers as compensation.

In FFXIV, you’ve already paid for admission. They don’t really expect you or need you to pay more.
The mindset really needs to be this: you are paying for a monthly pass, like you would for Disneyland. If a particular day is closed for maintenance, you are not compensated. Nor are you compensated if a particular ride is closed for the day. Your ticket only allows you access to the park when it is open, which you agree to when you buy the ticket.

Nobody forced you to agree to this, nor does anyone force you to play this particular game. You can protest sure, but complaining about it is moot.
The only free game time compensation I specifically remember getting from Blizzard was for the Wrath of the Lich King launch (it was extremely buggy with servers going up and down about every 4-6 hours for most of the first week). They gave us either 2 or 3 days (can't remember which). I vaguely remember getting another free day added, but I think that was more promotional than for downtime. If they offered any free game time as compensation after Legion, I have no clue since I stopped playing at the end of Legion.
Game companies do compensate for unscheduled extended downtime that was their fault, yes. Endwalker launch is a prime example of where SE has also done it.
For a scheduled maintenance? Doesn't happen unless something goes wrong and the maintenance lasts far longer than originally planned.

Is this inane demand still going on? You're not going to get a refund for scheduled maintenance. You agreed to the Terms of Service, which tell you there will be downtime for any number of reasons, and no you wont be owed any refunds because of that. Just installing and updating the game on a PC can take 2 to 3 days. And unless you're on a free trial, that's 2 or 3 days as a paying subscriber where you can't play the game... because you can't even update the game on PC without an active subscription. In fact, 2 days of maintenance for a major content update is nothing compared to what often happens in the industry.
WoW will often go down for days at a time, multiple times a week for a month or two after a major update. No refunds for time you couldn't play.
City of Heroes had update cycles where the game was either down or unplayable for nearly a month. At one point an update broke the game to the point it would crash after 5 steps or 5 minutes, whichever came first. No refunds given for time you couldn't play.
D&D Online: Stormreach tends to be down for a week or two all told due to hot fixes any time they release major content updates.
DC Universe Online has the same issues with update downtime.
When I was trying to play Eve Online (which requires a paid sub), it was down for maintenance more often then it was playable.
Whining about a 24 to 48 hour maintenance window is laughable.
