This has gone back and forth a few times, so I looked it up to be sure.
Odin killed Urth (and Wiyu, for that matter), and was then sealed by the Allagan Hero.

it could be surmised that as Odin slew Urth, her last wish and efforts to seal him proved successful. Odin being enraged at this trick may show his grudge ever continuing.

This actually makes sense. I mean when I finally got to fight Odin I was really looking for him for weeks. Wow, now I am even more of a douche. Still, who were the originals?

Don't forget, Answers was written well before Yoshida asked to bring Dalamud down and have Bahamut break out of it. It's quite hard to pin down what the song is actually about because it's quite easy to retrofit it to lots of sentiments. The biggest clue we have is that the word forsaken appears in it, which is a word Koji Fox has said he's worked into many primal-related things.
Odin's mask only comes on when he's engaged in battle. If you can find him unclaimed, he'll be unmasked with the face of the player that's his current title. If you find him unclaimed without a player title at all, the scarred Elezen is his default appearance... if I remember correctly, always an important subclause, lol
I did find him alone, but it was dark so i couldn't really see the helmet.
As for Answers, well likely it originally referred to a lot of other things, but later, Bahamut's story was worked around the song. Think about it. In ARR we only hear Answers during the coil, with remixes all over the place, and the actual theme during the Bahamut Prime battle.
It is his theme now. He is a he, right? The song had me thiking, but B is referred to as a dragon king, so...yeah

Well, kinda. It's the theme of the seventh umbral era now, and the Coil story-line was meant to wrap all that stuff up. Still, it's a damn good fit through the eyes of a Mericydian.
I wouldn't spend too much time trying to fit the lyrics of Answers into an game event. I like it, but it's essentially a lamentation of the fact that life is set of struggles with death at the end, and how even knowing that, people still stand up and fight. Very common in FF series (FF9 springs to mind).
Well it has supplied a new basis for why Odin keeps appearing.
That Odin is the Sword, the weilder is just the tempered person who last deteated Odin and took up the sword. That the sword contains enough Aether to temper the wielder. The duskwright Elezan was probably the first one to find the Sword after it was freed
