Almagest from neo, especially that one right after Omega always scares me to this day, but overall was just a pain in the butt when it was relevant....
Getting the Misdirection marker in Guardian and then being told not to move when you want to move out of the way from being shot to death.

In all seriousness, as easy as O5S is at this point the RNG with ghost boxes and the tethers is an ongoing nuisance to me when people forget to clear out most of the boxes. At least playing SMN gives me some damaging options while I keep moving. Getting stuck with an Air Force add on the other side of Guardian is also a nuisance since I have to wait to make sure Guardian isn't hiding any of the missiles behind it's bulky robot body before making my way over to kill the dang add.
Line of four ghosts in Phantom Train (where you have to run way back just to avoid them). Combo'd with other mechanics and you're just going to have a really bad time :(

Honestly, the savage train is just a mean clusterfuck for casters and healers in general, especially BLM. I should just...not even bother placing my leylines anywhere unless I can stay up front for something and then again, not even. :|
I have not raided since the Alexander series completely burned me out the game, but the savage mechanic I hated the most was the A3S Tether swap. So many wipes due to the tethers deciding they didn't want to swap.
Clear parties on the Chaos datacenter are easily the most difficult mechanic and you have to deal with that one every tier.
The meanest savage mechanic is actually the parties in PF that think they're ready for it but really really aren't for one reason or another, and then aren't willing to accept criticism.
I'd probably do Savage more if PF wasn't so willingly bad.
