1) Out of combat MP regen is so high that the mana used to cast stoneskin while waiting for a pull is totally inconsequential. I don't mind casting it early to ensure that most of the group has SS even if the tank pulls without waiting on buffs. 6/8 people having stoneskin on a pull is better than 0/8.
2) There are some situations you would see me cast Medica II 'unnecessarily to heal 1-2 people'.
-I expect periodic light aoe soon after, and I'm getting Medica II HoTs up to take care of it completely so I can focus on other things.
-There will be some damage that makes the HoT useful, and I want to burn a bit of extra MP so I can use shroud of saints sooner because I expect a roughly 4-5min fight. Using it early means I get to use it twice.
-I anticipate high movement with a good chance of people taking damage, so I put up Medica II to give people a higher chance of lasting till I can patch them up.
3) Either swiftcast is down, or there is no excuse.

Simple, they don't know and some one should take the time to tell them politely. Perhaps they might get offended but if done politely they might appreciate it a lot more than just calling them dumb or dismissing them as lazy.
Something like:
"[2nd healer's name] pls can u raise me asap when I fall? [state reason here so they understand why]."
"You can save more mp if u wait till they are out of their cutscene [state reason here]"
Anyways, whenever some one tells me advice I prefer to thank them so from my perspective it seems logical to just tell them what the problem is. -

1& 2 are pretty much explained by ppl BUT
3) is cuz if lets say, the boss is about to do some crazy dmg and ur 50% mp. R u gonna rez or heal THEN rez on an opening.
I have seeeeeeeen so many healers insta rez dps just cuz they saw they died to only have gone Oom and be useless during the raid wide dmg or tank busters.
Good example is during atma linga in rav ex. I rez during atma linga so tht by the time they r up they will have breathing room before the next stomp. Ppl forget they have invincibility after a rez so ppl derp and kill themselves.
I also would rather heal up the party ,asses my mp and then decide if the rez will really help.
Also bards/mch r part of the reason y making a decision is hard as 8/10 will not use their mp recovery skill till u Spam chat. -

Unless there is something really specific coming up I don't need to hold all my mana I have loads of it and two ways to regen mana. A single medica can often crit and will heal most damage to more than one party member. Spaming cure over two or three people is a waste when I could be doing more with that time.
3. It's one of two situations. I don't have swiftcast ready and I'd rather heal the people who are still alive and make sure nothing goes wrong since I'm now the sole Healer since your stupid ass got yourself killed. If I need you now I'd PoM and try to get you up. The other situation is you are really annoying whining in chat that you want a res when you derped into something and in that case you can eat floor for all I care until I need an extra Healer. -

2. Depends on content. Doing it is a good thing if the healer dps' s on normal dungeon. A good healer knows how to manage mp and dps. Rather than heal and stand there.
3. Dumb, if what you said is correct. Rez cost a lot if his\her mana was below a certain point. Ya if the fight is easy probably. Was waiting for swift cast. Cuz it wasn't a big deal. Or the person panicked. ..
