I have had 99% on multiple occasions.

A couple days ago I was farming atma with a FC member in Upper La Noscea. I got the atma pretty quick, but decided to stay and do the fates to help out since soloing fates gets rather boring. I ended up getting 3 of the same atma... She didn't get any.

Dont speak about RNG to me, when I meld or something els, and I see 35% success, you'll be sure I'll fail 12-15 times, it appennend already 3-4 times.
And then on next materia with 10%, take me 3-4 try only... Where is the logic ?
Also, once I tried to gather something new, 25% chance (and 75% when I had enough GP for the buff), I finaly success the gathering after 37 tries (at least 4 or 8 try with 75% in)

Atma was also a nice exemple of RNG or frustration, or fate popping, at least, with Atma, I cant complain, get normal luck I guess.

But for the fates for the book...
This is a true story, appen to me 3 times with the books :

I wait a fate for 3-4h, I go afk 2min to make a cofee, I come but, the fate is spawned and it's done, or I'm too late (75% done), cant get max reward.
Second and 3rd time that appen, I said to myself " I cant be that unlucky ? I can make a cofee "

Best RNG was a 14% HQ on an i95 primal weapon. Even a two 3 star mats that came out as HQ at 3%<----which is still high than an atma drop. Needless to say the 98% 3 star fails are rare but happen. I find with the 3 star crafting 85% ish has a 50/50 chance of nq. Don't have these bad of numbers on one and two star items.

I like to desynth summoning bells in batches of 10.

Keep in mind I have a level 100 BSM for this, so the success chance is at its maximum (70 %).

My last 30 summoning bells have been averaging 7 out of 10 fails, or a 30 % success rate.

Oh, and since people like to mention this sort of thing! My record for most consecutive missed Hasty Touches under Steady Hand II is 9 in a row.

I've done a master token craft, landed 5 quality below cap (out of some 4900) and it was still 98%. Unfortunately I don't have proof, nor can I easily replicate the scenario (or replicate it at all), but for all the crafts I've done I've never seen a 99% (I have the GSM Luminary too).

gather 5 HQ mats, in a row, with 14% chance success rate.
getting my 4th twin atma...cause i did NOT want it. was just doing some fates while waiting for mark-fate, when i though, oh well, already have 3 of these, don't want another one, but i'll probably get one, just cause i dont want it, and i got it.
a FC member did hunting for few days, and apparently got 40+ logs.
crafting HQ items, with 1% success rate.

failing to do stuff, 5 times in a row, with 95% success chance.

the bad

made 3 spicy tomato relishes at 98% and all 3 were NQ

the good i got atma weapon in 1 day within 12 hours of fate farming from atma 1 til atma 12

told me friends about that and never saw them for days already

maybe got heart attack?
