12 button mmo mouse or swapping to a controller for sure.

Oh boy, if you think FFXIV jobs have high APM, I would hate to see you play any other game that has a faster GCD.
Even with OGCD's, FFXIV's gameplay is still relatively slow compared to most games out there. It's something you get used to, and I've come to appreciate the slower GCD vs mechanics playstyle rather than the less mechanically heavy faster fights that other games have. But outside of movement it's still a rather slow game from a strictly combat/button pushing perspective.
Even if you double weave the the entire first minute (which no one does), that's three actions every 2.5 seconds, or 72 APM. That falls between "casual" and "experienced" for SC2.
Fix your keybinds or buy a gamepad like the Tartarus, which is what I did because I'm getting old.
More than that, for every instance of play, you can add 2, in switching from the boss to a dps, then back to the boss, 3 times. AST's APM isn't consistently high, like many jobs in the game, it's low outside of burst windows and gets higher during bursts. Unlike what Ransu claimed, AST's opener isn't "extremely low" APM.
I have all the melee dps at 90 minus Ninja and I dont think I will ever take that job beyond 70 or wherever I left it off at because I just dislike how clunky it feels with mudras. I don't count my SMN because....well its kinda brain dead easy to play.
The most "intense" opener is not even above 50 CPM. Sorry, not sorry, but the APM requirements in this game are extremely low. Everything in this game can be done easily on a controller, so if you're struggling with keybinds on a keyboard then switch to a controller. Otherwise stop clicking and start using keybinds because not a single opener in this game is super demanding. Even with all the oGCD spam this game's combat is SLOW.
This is true for most jobs but having played drg personally in high end I know how busy it can get in its rotation (the opener is not that busy tbf, busy part is life of the dragon)... I wouldn't like for them to reduce the apm too much tho I'd say maybe change it so the new lvl 90 skill takes 3 charges to use and rise the potency, but that's what I feel makes it so busy anyways.
How did I solve hand pain? I use Corsair K70 Mk.2 RGB Low Profile. This keyboard is extremely sensitive. Rapid fire is a boon. It is prone to typing errors though. Have to get used to it at start.
For long period of using Dual Sense and Xbox controller, I just lay it on my table or thigh. Your hands is not designed to carry weight continuously.
My old keyboard was a k70. It had a crazy high actuation point. I liked it at first, but those keys would actuate when you barely pressed them. Started to drive me mad lol.
Ok, so you get to add 4 whole actions the first 30 seconds, then 2 actions each 30 seconds after that, for a total of 6 additional actions for the first minute. 78 APM. wow such big, very busy.
Except it isn't, and that burst window doesn't even last a full minute.
Fix your keybinds. If you are doing acrobatics to play, you have badly setup keybinds. If you're old and decrepit like me, get an assistive device such as a Tartarus or a MMO mouse.
Can we get a full healer role rework to prevent hand injury instead. All this 1spam gave me a RSI and fixing that would contribute far more to avoiding strain than reducing APM as a whole.
Not to mention save me some money by not making me replace a keyboard every few months due to exceeding rated key presses. Well, not that I have that problem anymore since I quit the role altogether, but it would save others some pain at least.
