You don't need a tooltip to know why you're failing. You're failing because you didn't use enough synthesis actions to complete the item before all Durability was consumed.
How many more did you need? That depends on what you were trying to make and your gear. Look at the crafting interface. It tells you what progress you'll make at 100% efficiency. Look at your tooltips. They tell you what the efficiency of the action will be. Do the math.
What should your rotation be not only to complete synthesis but to increase the chances of getting a HQ item? Right back to it depends on your level, your gear and what you're trying to make. You might not be able to guarantee HQ on an item even using HQ mats. That's okay. Unless you're trying to complete a class quest that requires it, items don't have to be HQ when you're leveling. Your XP bonus is based on the increase to quality made through actions, not whether or not the item ends up HQ.
There's a reason why macros shared online list the difficulty plus minimum CP/control/craftsmanship requirements. There is no one size fits all crafting rotation in this game. This is not WoW with its one button, zero difficulty, gearless crafting system. I have 3 level 80 omni-crafters but I can't use the same rotation on all 3 because their gear sets are different. I can't use the same rotation to make both Pliable Glass Fiber and Facet Coat of Gathering because the difficulties are different even though both are level 80** recipes.
But I was perfectly capable of figuring out rotations and understanding why initial attempts might have failed because the information I needed was right there in the tooltips and crafting interface. I did the math and figured out what went wrong.
If you're bad at math, you're going to struggle figuring out crafting on your own. I definitely recommend using Teamcraft in that case. It's a good tool even if you're good at math but want to figure things out quicker.
