I leveled a character from scratch up to 99 and Bastok rank 6 some time last year on Bahamut. I didn't have trouble with any of the bottlenecks you mention.

The three mage gate you can frequently find help by /yell, or you can ask your linkshell for it. People are pretty eager to help because there are always a lot of bored people waiting for something to do.

The thing is, they changed the gates in eldieme and garliage and also allowed players to click a ??? for level 50 cap items.

By doing this they are admitting there is a problem, and so leaving these others makes no sense.

The only people who aren't there by choice are the people who created characters at the beginning of the game when you couldn't choose your server.
