A game called Aero Fighters for the SNES. If you can even make it to the last boss its insane.
But, that is a str8 up n down shooter, old school plane one where you have like 50 million bullets flying everywhere, and you have to dodge them.
Ye that is hard.
The guy dropping the blocks on Tetris. I've never even made it to him. Everytime my wall gets tall enough to maybe reach him, I get game over. :(
In my personal history, I couldn't make it past The Four Kings in Dark Souls, on NG+. It was too frustrating to keep trying over and over, so I quit...which I rarely do with a game.
Definitely not Twintania or Titan Ex :P. In a MMO tho I'd have to go AV :P
well, there are a few games that were tough, but that was just poor programming (pretty sure Angry Video Game Nerd has several games he's reviewed to that regard).
Then again, sometimes, people mod the game for challenges.

Ninja Gaiden 1 last boss on PS3...
In MMO's, def AV before 75 cap was removed.
That was just ridiculous... we had 200k people theorizing how to beat it. Its also the reason why us, the community, finaly figured out that SE DO troll us - with that idiotic video of their PLDs using earth staffs to tank AV and the cameras zooming in people using their two hours.

b.) Thought this guy was LITERALLY psychic

AV was quite the boss. The theory crafting, 1000s of attempts, and huge threads where LSs actually traded strats and secrets.
Absolute Virtue, no doubts. Unfortunatly I doubt we will ever see something like that again. Once in a lifetime, eh?
Lunar: Silver Story Complete. Just about every boss was a pain because you couldn't just level higher to beat it because they would level with you. I was also young and didn't know how to figure out mechanics. Probably the only RPG I quit without beating.
Not an RPG like everyone here is comparing.. But final boss of echo the dolphin for the sega genesis. 40min long stage, no checkpoints, no health recovery, millions of super fast projectiles being shot at you.
Error 90k can be a bitch to beat!

In MMORPG, I think probably heroic Ragnaros... I've not even beaten his normal version.
Demi-Fiend/Hitoshura in Digital Devil Saga was a pain to beat.
I guess one of the hardest bosses I have ever fought was Emerald Weapon in FFVII. But I wasn't capped, and I didn't have Knights of the round...and it was like a 4 hour fight, where i just kept summoning Phoenix for as long as I could, basically I didn't have the proper set up, and it was a darn good fight :)
Which I eventually won :D
Also hardest game I've ever played...Resident Evil - Hard - Chris :)

off the top of my head the only boss in a RPG i can remember having difficulty beating is Dark Bahamut in FF IV. I'm pretty sure i almost broke my GBA in a fit of rage.
EverQuest: Kerafyrm, The Sleeper. It took over 180 people at the time of the first kill, many people lost several levels, took over 3 hours.

And they won with an exploit to boot. Pretty sure that event wasn't supposed to be completed though. And from what I heard it ticked a lot of people off because it made the event before it go away.
I'd say you're probably going to see a lot of "hardest bosses" from the 8 and 16bit eras of gaming. Whether it's due to game glitches making it difficult or just plain old difficulty you won't see stuff that hard again unless it's a majorly niche title.
The hardest fight that I can recall for me though was in Valkyrie Profile for the PSX. I don't recall the name of the boss but it involved going back in time to a city's past to find out why it went apocalypse. The boss fight you have to go up against there is very tough if you didn't expect it as he dropped a very powerful AE on your party very frequently.
Probably some sidescroll space shooter on max difficulty with projectiles from every corner.
Shredder from teenage mutant ninja turtles arcade nightmares of him splitting into two and his life points are crazy, remember popping in quarters just to beat him and it took forever.
The other is Gill from Street Fighter 3rd Strike.

It was pretty much the biggest zergfest of all time. Clerics pretty much sat there spamming res from their Epics. There was no exploit, but he did bug out and despawn at 25%. GMs were called, they agreed it was a bug, so Sony agreed to reset the even and posted an apology. Sleeper can only be awoken once ever, per server. Can never spawn on that server again.
I was lucky to be logged in when he was woken on my server when I played during that expansion.

I'd say The Sleeper back in EQ. Think it was like 2003 when he was first killed with something like 150 players.

Ragnaros wasnt particularly hard... Hmm.... Firelands was ok, not that wooo impossible hard (rags included)
However.... Hmmm.. I can't think of any that I played that was particularly hard enough to earn this title... :(
I've played a bunch of games and a lot of them had challenges in them but... Well, I wouldn't know! xD
Try as hard as I might, I was never able to defeat the final boss in SaGa Frontier 2, even when I replayed it years later.
In terms of MMOs, WoW had some pretty difficult boss fights. C'thun, M'uru, 0-light Yogg-Saron, heroic Lich King, and heroic Ragnaros come to mind. They make the difficult fights in FF14 look easy.
