Falling off map

Twice now I have "fallen off the map" Once in central shroud close to the south shroud entrance, and once in Limsa Lonimsa on the way to Hawker's Alley. The character just falls off the map to the underlying area and I can still see the map above. There is no way to move and the mouse stops functioning. Only when you click does it ask to "Exit Game"
