I remember reading it as you 'Normally' attach materia 1 by 1 (only 1 piece of materia per gear?).

Attaching materia requires the “materia melder” key item, an apparatus which can be obtained via a quest available to Disciples of the Hand level 18 or above (with the exception of culinarians and alchemists). The apparatus allows players to attach one piece of materia per item.
However you can conduct a special 'Forbidden Materia Craft' using a different 'device' to the one used in normal materia addition that allows you to put 5 pieces of materia on. While it goes on 'all at the same time' with one use of the device, you attach each piece individually during the process (think of crafting. Your bar goes up, item complete, then another bar appears to work on the next piece right away until all 5 are on).
If you fail on a single section of the 5-piece materia adding, then you lose your all materia involved in the process, the catalyst and the item (e.g. Bronze Gladius would be 'item'. The 'Melder' is the key item and won't be lost).

I'm pretty sure I read that Ex gear can't be turned into materia and that it also cannot have materia added to it, to keep normal non NM/Raid gear on par with them.
