Dunno if this is 100% accurate to what you do, but you should be in Defiance before the pull, pop Infuriate, then Unchained as the PLD is pulling, and then HS etc.

For P4, assuming you're not tanking for P3, you should have Unchained available for this. Ideally you want to be dropping Defiance when only one add is alive as it's significantly less damage. Don't play too risky in this phase as there's 3 sets of adds as opposed to the much simpler 2 in P2.

Again, shouldn't be tanking during this phase so should never need to be in Defiance outside of Tether -> Charge.
Other things:
I.) As Raylan mentioned, the HP threshold for WAR in this turn isn't actually that high (you need to survive Charge and the adds, that's basically it) so swapping to STR accessories is perfectly fine.
II.) Last phase you'll want to pool your STR pot to use with Berserk as it's the last STR pot you'll be seeing for the rest of the fight.
III.) If you start running low on TP in the last phase, drop Fracture.
IV.) If you did have to tank for HL -> Charge, then what you're doing is perfectly fine. Ideally, for DPS, you should never be tanking, though.
