I finished Sastasha and ended up walking out of the water in the screenshot, not exactly where I entered, so I was rather confused.

Not a bug.

The first time you complete all of the new dungeons you'll be in a different spot after watching the cutscene. Many fights in the game are like this the first time you clear them.

If you check the screenshot you can see it's not Sastasha HM, it's the level 15 one, it shouldn't happen.

Did it allow you to leave the area?
That is defiantly a bug you shouldn't be on that island until around level 40 or so.
Thankfully there are not mobs by the boat there or you would have been in some trouble.

If I see right you are standing on the Isles of Umbra, which should be unavailable to you until your level is around level 42 if i remember correctly.
