Random game crash

I have been having a random game crash not sure what is causing it but it's just out of the blue. Sometimes happens when teleporting, walking, running, jumping, breathing...just random. Here is the crash information from Windows:

If anyone has any information or if I can get some help from a square tech person I would appreciate it. Thank you.

Maybe a reinstall of the game would fix this? Any suggestions anyone.
