Hrothgar has some helmets their heads wouldn't even fit inside :P.
I think that the moment we start thinking about this it kinda falls apart immediately, imagine how bad it'd be to wear a helmet with horns too.
They're still attached to your face if you're an Au'Ra a blow to them would still hurt you or worse and people could just grab on to them and fling you around and they're out with helmets too.
I remember in the Baldur's Gate 2 Beta/ Alpha one of the demons in the tutorial has wings and wears armor but the armor only covers the front his entire back is exposed to give way for the wings.
These things are just complications altogether.
I dunno why in DnD that tries to be '' grounded '' too Tieflings often have their tails out, if characters in the setting don't seem to care then I dunno why we as the players should.
Maybe it'd be really uncomfortable to tuck your ears down under a helmet but at least you wouldn't lose them.
Like maybe you could tuck them back in a backslip and bind them or something.
