If that dumbass tank would have used Defiance, he would have had the stacks for Inner Beast which would have saved him from Final Sting.

Alright. I got one. Might not be a "horror story" to more experienced healers, but to me it was a mess.
Last night I ran the Vault for the first time on my WHM (I typically Tank, not heal). I'm still a little gimpy geared for it. Told the group. They were cool. Got to first boss with everything going smooth. Do the first pull at the top of the stairs and the Tank's Hp start dropping like crazy. I'm spamming cure II but can't keep him up. I Pop bene at his bottom 10%. He still dies, and I'm at a loss because I've literally used everything I could to keep this guy alive.
Found out it wasn't my fault. It turned out he was Dc'ing and my casts weren't actually healing him. One of the Dps were in voice chat with the guy, and said so... just before he bailed.
K. No prob.
The last Dps and I votekick the tank and look for party members. We get a Dps quickly, wait for 10 min, still no Tank. The Dps get edgy and decide we should continue on without a Tank. I end up having to heal everything from the second pull (that includes the top of the stairs, since nothing had died before the first Tank Dc'd) all the way to the second boss room while playing the role of paper tank with the Dps (those giant knights and chess piece knights hurt, by the way).
We get a second tank just before the next boss... who proceeds to stand in almost every AoE the guy televises, turns cleaves towards the Dps and me (wanted to make sure I was awake, I guess), and then speed pulls the rest of the dungeon all the way to final boss with almost no Cd's. He pulls the final boss before the Dps are even at the door, and then proceeds to stand in the charge line of every single Knight and is almost constantly slowed or bleeding. My Mp was sucking empty for the entire last 25% of the fight. Fight ends. Tank busts out of there before the loot box is even open. Dps speaks up and says, "anyone else think that run was going smoother before we got a Tank?"

Something I don't understand is why do people queue for In Progress if they are just going to be a jerk when they get into the instance? It's ridiculous.
I think you misread it slightly, the tank didn't bail, but the tank's dps friend did. They had to votekick the tank (for being offline, I assume he didn't come back).
Regardless, people probably queue for In Progress in hopes of an easy roulette bonus, hoping the group is at the last boss or whatever. Less work for the same reward is all the incentive people need a lot of the time.
He didn't. We were waiting for a few min before the Dps friend admitted that he knew the tank and explained what happened (which was a relief, because it meant I didn't kill the guy). We said it was fine and would wait, even had a nice conversation going for another 5 minutes or so. During the chat the Dps who knew the tank up and left without saying a word. Me and the remaining Dps figured that the tank wasn't coming back at that point (given that his friend didn't wait around for him), so we started looking for members.
Actually the paper-tanking was kind of fun. It got pretty exciting when we started fighting the knights and the AoE dmg was getting heavy. I don't think we would have survived the boss though. His cleave was taking out ~70-80% of the Dps Hp bars (which I found out courtesy of the 2nd Tank's penchant for feeding Cleaves to the group). I managed to keep everyone alive, but I don't think my Mp bar would have lasted the entire fight with that kind of dmg. Hard to say though. Part of me kind of wanted to try... especially after the crap that Tank put us through.
You're right. I'm guilty of it myself (short runs to cap tomes or get a quick exp boost is always nice), but I have never entered a in-prog aiming to be a jerk.
If I'm entering an in-prog, then something has obviously gone wrong (hence a missing player). I don't think it's cool to make things worse. Besides, it's a bit of a gamble to act out in a in-prog group. You never know why that spot was empty. The group could have already vote-kicked someone for being a jerk. What's one more? It almost happened in my group as well. The Tank we got almost wiped us several times over by kiting AoE's and Cleaves into myself and the Dps. Right off the bat I was considering another vote-kick, but I hesitated.
I main Tank, so I have high standards. I admit that I tend to judge harshly, and for that reason I actively try to not criticize unless I absolutely have to or someone else in the party says something first (thereby vindicating my harsh judgment). In this case, neither of the Dps said anything, so I let it slide. By the end, though, they both piped up. It turns out that none of us were happy with the replacement. If we had been just a bit more vocal during the run, the Tank would have gotten the boot with a unanimous vote.
Paper tanking can be fun but I've never done it at the Vault yet so I'm not entirely sure how hard the mobs there hit. It just sounded like more unneeded stress, but as long as you had fun... xD
About In Progress: Yeah, I always try to be as nice and helpful as possible when I get an in prog because I don't know how Vote Kick happy the group is.
I'm trying to learn to be more vocal when something is going wrong, or someone isn't doing a good job (outside being new). I am more hesitant to kick Tanks though, primarily because it's such a long queue time. I've even had tanks who knew they were being an ass but taunted us with "You have to keep me around because you would wait forever for a new tank!"
The trash mobs hit pretty weakly (so long as you don't grab a huge group of them), but the Iron Giant guys and the Chess Piece Knights can hit pretty hard if you're not on the ball. First set of Party wide AoE's caught me off guard, but if once you expect it it's not so bad. If I can do it at my gimpy item level (still rocking a few Ironworks pieces) than I'm sure a better equipped healer could do it no problem.
In regards to Tank ques, I have a bit of an advantage. I Main as Tank, and have all 3 of them at 60 and geared to the teeth. So, I have lots of experience tanking and can swap out whenever I need to. I've had one tank pull the "you can't kick me" stunt on me once while I was running as my Drg. To which I replied: "I'll tank" and vote-kicked the guy. I actually kept tanking as my Drg until we got a new tank. I told the group that I'd pull out and join in prog as one of my Tanks if we didn't get another one by the time we made it to the next boss. Thankfully we did, and I got to keep my exp, but if it came down to it I would have switched. No one should have to put up with that kind of garbage.
Rolled Pharos Hard for DREx, loaded into dungeon, got a newbie bard and blm. First signs of trouble arises at the 1st boss where no one took adds off me. No issue, popped my macro for tank to take aggro and we passed. Reached the 2nd boss and that's when all hell broke loose. No one bothered about killing gels above the vents. DPS doesn't kill, tank doesn't kill, no one bothers taking them off me either. I lightspeed/gravity the lot too late and died when they exploded. Wipe. We restart. I tell the group to kill the gel on top of the vents. Checked DPSs language, repeated instructions in Japanese. No replies, feeling a bit apprehensive already at this point.
What do ya know. AGAIN no one kills the gel, saved for the difference is that now the bard drags the gel over the vent but doesn't kill it? I Malefic it too late, gel explodes, taking out the bard. I ressed the bard while running away from gels and an annoying Be Gu cannonballing himself around. Boom, 2-3 gels take me out while I was standing in place healing the tank. Third try. This time, I told myself, i'd kill the damn gel over the vent myself if I have to. Vent spawns, gel spawns, I whip into cleric mode, grab gel, drag to vent annnnnd, bard comes running to me with another gel! She doesn't kill it again, of course, just stands there taking dmg from a blob wailing away at her. I survived the gel exploding on me, but Be Gu's cannonballing took me out. All the while, the BLM just sits in her little comfortable corner, DPSing the boss.
Respawn, someone initiated a vote abandon. I clicked a YES, relieved to not go through this living hell anymore and just as I was loading out, the bard finally speaks and says "WTH was that AST doing?"
I've never ever wanted to throw my keyboard at my monitor so badly in my whole life.
This has a ton of details, so it'll be split into three posts:
Earlier today, before maintenance, I was running Void Ark with some FC members and an outside friend. When I do large raids, I usually gravitate towards AST because I can choose to be more SCH-like or WHM-like as needed. My FC leader, who is a SUPER TALENTED WAR, decides to be MT as he always does. (He dislikes "beta tanks" and this gives me more to do in raids than just derp around.) That being said, he later told me that part of his lacking at some points, namely the final boss, was due to some shit while tanking, totally understandable because it's VERY hard to do anything and MT a 24 man raid at the same time... Speaking from personal experience from the Crystal Tower. All of our other members that queued with us are DPS. My cohealer ends up being a WHM, which is perfect-it means I can be Diurnal or Nocturnal to better suit the situation's needs.
The first trash goes smoothly, and the 1st boss is an okay run-through. I noticed that my cohealer only used Esuna on himself. The next trash/etc goes pretty smoothly. Then, we hit the tentacle dude. I use my Diurnal Sect (for obvious reasons) and heal like normal. Everything goes fine until my cohealer does a pathetic job of keeping himself alive, even with my help. So, not only am I healing the party, I'm reviving him roughly once a minute, and since my FC leader is main tanking, so I have no choice but to keep reviving him. (I don't think I can solo heal that boss, especially with 4 ranged DPS that hate standing together.) I eventually died to tentacles because my whole alliance was dead, excluding my tank, and I had to LB3 in the center of the room in hopes of hitting everyone. (On the plus side, that was my 1st time seing AST LB3... it was BEAUTIFUL.) The only thing my cohealer did right was resurrect me, but I died soon after and we wiped.
Afterwards, we managed to get past that and reach Echidna. That fight was a disaster. One tank did not keep their snake away from the other snake. On the second attempt, we get everything done right, for the most part. However, remember my comment about the outside stuff interfering with my tank? He was petrified once by the add, and vuln'd two other times after that; one from being petrified (only a few seconds left on his first debuff+interference occurring) and one from Abyssal Reaper because of his petrification. (Some tanks don't know that you should stun Petrifaction during the add phase.) He hit sub-3k HP (from about 20k) because my cohealer was *totally* doing his job while I was picking up an alliance member. Luckily, he knew what to do as far as spacing cooldowns Did I mention no Benediction? I picked up majority of that missing HP on my own. Oh yeah, we have a new inside joke: I'm the Main Healer.

I'd agree with you, only during the charge phases of that boss fight anyone who hits the boss stuns themselves and cancels their Dps anyway. Might as well just deal with the slime and adds instead of wasting time trying to hit a boss that is electrocuting you for every hit. The faster the mechs are handled, the faster the Dps can get back to the boss.
I can kind of sympathize with the tank not pulling the adds since, more often than not, the Dps kill them before it's even an issue (no point in wasting Tp/Mp on an AoE aggro grab that sacrifices position for the sake of adds that will be dead in under 5 seconds), but the Dps sound like total trash here. You have two ranged Dps who could have easily handled the mechs no problem with minimal interruption to their rotations. There is no excuse for them not doing it. They already have the easiest job in the game, laziness as a Dps is not something I can easily tolerate.
In a 24 man cluster fu... I mean raid... I never trust other players to handle mechanics correctly. Instead of trusting in a stun, I just focus target her and look away (like in T7, back in the day). It's easier, much more reliable, and typically handles itself anyway if the other two tanks are keeping the snakes away from Echidna. If you're attacking a snake, chances are your back is turned to Echidna... unless you're a Tank or Healer, that is, but that's what FT is for.
Either way, sounds like your co-healer was there for a carry... or was very noobish (in which case I'm sure the Cuchulainn fight was rather shocking to him/her). My FC healer tells me that that fight was not fun the first time she did it... That being said, she also managed to solo heal 2 out of 3 of the parties while the healers ate the paste or got themselves locked out by accidentally respawning instead of accepting a rez. At the time I laughed at her expense, but given all the times I was forced to near-solo tank the Tower Saga, I sympathized greatly.
I have a few stories, but I will share the horror I encountered today. Went into leveling roulette with my lvl 57 SCH and got The Aery. I love this dungeon! Admittedly, I'm still learning how to properly heal and DPS at the same time. AST is my main class so it's a bit of a change for me. I love it, though!
Anyways, so got in, everyone said hi, tank actually let me cast everything and even let me get Adlo on him :D Go to the first pull, I put our a shadow flare as the tank seemed to be sticking to one place. DPS is doing good, I'm doing good, tank seemed to be doing good... until he started moving onto the next group when this one was about a quarter of the way down. We were like ??? but went along with it. Eventually, we got to the broodlings and the slumbering dragons and he decided to go pull all of them and proceeded to die. (c)
I admit, I was a bit stunned because of his manner of pulling. Even the DPS were saying "I'm confused lol" and saying that they'd never seen that manner of pulling before. I asked politely if we could just pull like normal and I think the DPS even said to just let us know if he's gonnna pull like that so we're ready, to which he replied "Okay." I thought he was replying to me as well, but then he said "No I'll pull how I want to pull I know what I'm doing". I was like whatever, he's gonna do it whether we like it or not so I just gotta be prepared.
Well, we got to another pull and apparently I was typing when I could have been "curing." Now, I know it might have been a douchey thing for me to say but I just said, "I don't cure, I physick." Lol, a little healer humor but he obv. didn't appreciate it. And so he proceeded to tell me to do my job and to stop typing. It was hilarious. I told him he should be a more considerate tank and realize he's not the only one in the dungeon.
He left as soon as the last boss was down (which went well, thankfully) and I'm sure didn't commend. I got both commendations from the DPS though and was sad I couldn't give both of them one. The DPS and I actually worked together to get through a dungeon with one of the rudest tanks I've ever seen in my time on FFXIV.
Anyways, this is but one of my many healing horror stories and it really makes me question if I want to be a healer or not. I LOVE healing, but seriously... some people... make it so. effing. difficult. /disappointed
Tanks that go on a little site seeing tour whilst pulling are really difficult to work with :(. But at least you seemed to have managed to compensate for it with the dps as well :).

Generally I make fast exits as I don't think I was good enough to get comm'd anyhow.

I got another one. This one's a doozy.
So, for my Leveling Roulette this morning I got Aurum Vale (enough said, right?). The Tank and one Dps were watching the intro, so I asked who is new. One Dps said they were new. Okay, no problem. Second Dps says (no joke): "This is my first dungeon."
...Whut?
So I assume they just worded that oddly and say: "First dungeon? lol" To which they respond. "Never done a dungeon before."
...Whut?
AV is a lvl 49 dungeon. The Dps player in question was roughly at item lvl 115. Now, skipping dungeon runs on Dps is not uncommon (cuz of crappy que times), but to have never run a dungeon before? How is this even possible?? Anyway, at least the Tank is not new, so I figure how bad could it be, right? WRONG!
First thing the tank does is run down the middle of the 1st room. That's right, straight down the middle. To be fair, she somehow manages to not pull everything; however what she does pull she does not hold aggro on. I cast one cure (just one) and suddenly I've got 3 mobs on me. Worse yet, both Dps and the Tank are standing in the yellow crap that's all over the floors and are slowly killing themselves.
Wipe. Re-spawn.
I inform them to not stand in yellow puddles of death. Tank says "kk." Pulls again (more carefully this time), still loses aggro, and one of the Dps is still killing themselves in the yellow gunk. Joyous time to be healing/paper tanking. Somehow, we get through the room to the first boss fight.
Tank says: "I don't remember this." At this point I'm getting suspicious that the Tank is also new (to both tanking and the dungeon). I inform them to eat fruits after stacks of debuff. "kk," she says... and nothing. She just stands there. A Dps gets antsy and pulls the boss. I heal her as best I can and mange to keep her up through 2 cleaves. By the time it's the 3rd cleave, both Dps and the Tank have 4+ stacks of debuff. Paper tank dies to cleave. I swiftcast rez. Tank is now tanking. Other Dps dies to debuff stacks. I rez that one too. First Dps dies again (this time standing in yellow gunk +3 stacks). Dead Dps rage-quits before I can rez. Good thing, because I'm damn near empty on Mp. Other Dps dies again, and I can't save them. I (somehow) manage to keep tank alive till end with almost 0 Mp (casting cures as they proc at this point). We clear boss.
New Dps joins and says "what happened here?" To which I respond, "way too much noob..."
It got a little better after this, but not by much. Tank still failed to hold aggro the entire time, and stood in almost every single Bad Breath they could (cuz' I had the Mp to spare, obviously), and the remaining of the 2 original Dps refused to listen to boss instructions on the following 2 Boss fights, promptly finding a way to kill herself in every single fight we go into. I got them through it (still one more wipe in final boss arena, during which the Tank accidentally turned a bad breath into the entire party), but I could not get out of there fast enough. Definitely one of the worst runs I have ever seen. Makes my top 5 for sure.
It's not possible. Most dungeons unlock through Main story quest.
That's why I was so confused.
The only thing I could think of is either this person had literally been carried through every single MSQ dungeon in the game, or they were a troll. I was half expecting them to do okay (maybe they just had bad taste in jokes or something?) only they were the worse of the 2. The "I've never done dungeons before" Dps was the one who rage quit after feeding herself to the boss twice. I couldn't believe it. Given that this player also seemed to ignore simple dungeon mechanics as well (such as letting the Tank actually do their job, for example) I almost could have believed they were new to dungeons, but again.... HOW??
So I had a weird one.
I was doing my Low Level Roulette for tomestones and I got Sunken Temple of Qarn. Okay, whatever. When I started the tank was acting weird. I'm not sure how to explain it. He was moving around very slowly and seemed to be looking for something. He paused in front of the mass of rubble blocking the set of stairs to the left after the first pull (the set you use during Qarn HM), so I was thinking he was may have been confused on which Qarn he was currently in. Whatevers, he has all level 50 equipment so there shouldn't be any issues... Well he didn't know how to hold enmity at all. I stood around for a good 5 seconds or so t the start of some pulls waiting for him to use Overpower or something, but a lot of the time he kinda just floated around, attacking stuff at random. I would literally cast 1 Cure spell and I would pull hate from something. I wasn't the only one either. The BLM did it a lot, and the NIN too. One thing that sucked for the NIN was the tank would dodge AoEs but not go back to where he was and instead stand in his new spot, turning the mobs and bosses constantly.
We wiped on the first boss. The tank died twice from doom, the BLM did too but he apologized for it, the NIN tanked the boss for a good while. It was too much damage, and the bees didn't help. So we wiped. Next attempt the tank removes his doom without anyone explaining it to him. He must have just forgotten... Everything goes okay until the second boss, which goes fine but... The tank asked "everyone knows what to do with bombs right?" I "lol"ed, and the BLM relied with "lmao what?" It was strange...
The tank also didn't understand how the statue head things and the platforms work. He kept pulling the first one too far even when the NIN explained. He also stood around and walked back and forth between the two rooms with the flame of magic and fruit of knowledge without entering either. The BLM asked him if he was scared. He eventually decided the enter one of the rooms after about 3 minutes.
The final boss went fine. The NIN and I worked together great in this fight. We focused adds together and everything. It was cool. I gave him my comm and I got one in return.
It was just a really weird run... I don't know if the tank was new, drunk, high, forgetful... Eeh. I got my tomestones so whatever. >_>
Weird is an understatement lol. It sounds like your tank was confusing multiple parts from multiple different dungeons within the same run. Going the wrong way would be Quarn HM, and the bomb mechs for a Golum boss fight would have been CopperBell HM...
Either this guy was high as a kite, or was experiencing the onset of Alzheimer's or something... Very strange indeed.

Talk about an awful first dungeon run, then. I joked at the beginning that AV would be a terrible dungeon to get as a first, but I never actually expected the person was serious... If they were, then I almost feel bad for them... though not as bad as I do for myself for having to heal that mess >.>
If I see 1 more tank pull 3 groups of mobs and only use 1 cooldown while I'm trying to dps I'm gonna scream.
I had a WAR tank in Pharos Sirius HM that only used foresight during trash pulls and CONSTANTLY moved the mobs out of my shadowflare.
