Because Esuna is a non-mechanic after Stormblood or so. And players can't be bothered optimizing their ARR dungeon runs. Those are quite literally relics of a very different game.
My personal experience to this as a healer main was littered with trials and errors.
I remembered the first thing my intuition told me when I learned Esuna was to hover my mouse to those icons next to our party member list and read them to find the detrimental ones. From that point, it was all "press if it's ominous" then memorize which ones works and which one doesn't. It took me until my first playthrough of ShB back in 5.2 when somebody mentioned that cleanse-able debuffs has a little white line on top of their icons. Sadly it mattered less to me because at that point I was in middle of learning how little healing/esuna I can do and still get away.
I'm still doing this because it gives me something else to press. There was one time a random tank thought I was RP'ing when I kept doing it, and they were pleasantly surprised when they learned the /comfort can cleanse those Misery debuff lol.
My points were twofold:
1. First and foremost the game doesn't teach you how to use Esuna. Yes the skill says it cleanses debuffs but not all debuffs can be cleansed, it doesn't say that the cleansable ones have the silver bar above and
2. The game doesn't give you many things that are worthy of Esuna-ing making the whole cleansing mechanic confusing.
DoTs as I have previously mentioned can be regened over for the most part because they aren't threatening with some few exceptions.
Of all the debuffs we should cleanse there are three: Any slow (mostly slow and not heavy), any immobilization, and doom.
And as I said in the first part, not all of them can be cleansed. Damage down... depends. If I can heal through it, I'm going to heal through it. Same as any other Esuna-able debuff.
I was giving an answer to your question. As for the most part, it doesn't make healing any more difficult. If it did, I'd Esuna.

Doing Dun Scaith as SCH is great because it means big heals. Doing Dun Scaith where our tank decides to take on Diabolos Hollow as SCH is torture because my co-healer never does more than the bare minimum to help and I run out of resources and our tank dies.
(Though, to all tanks: don't tank him and also step in the stack marker if you can help it. You're already taking chunky autos, you can't afford to also take that damage. Most MTs die at the very end because they do this.)
Yep. That and the enemy cast bar flashing meaning it's interruptible were added long after launch, so there's no tutorial for them... but they SHOULD be added. I cannot tell you how many times I've seen a sprout tank failing to Interject at an important moment. And who could blame them? How should they know this?
They do just ignore it because they most often don't know it can be cleansed. Since it doesn't have the blue bar. Once in awhile I do have someone ask how you get rid of it or why even as tank am emoting the dps as misery can be cleansed by anyone since it's method is an emote.
Thank the twelve that you don't run roulettes often or when you do that you don't get Dun Scaith. The ever self perpetuating circle of screw this I'm leaving because no one knows the fights because people don't stay to learn the fights. And or try and teach. And no I'm not talking about the death puddle that people don't keep on running from after the 1st pull in, but the 2 Esuna-able dooms the boss puts on random people in each party. Yes, I know old content. Not Esuna-able but any time I get SoS I worry about the opening few seconds to see if the healers realize wtf Elidibus inflicts on the party that drops the entire party's health to one. Cause I still remember the one run where for some reason most stayed after the 5th wipe thar was caused by his True living dead attack. 1st 3 were from not healing everyone to full and the other two were somehow not healing the tanks to full. This was late ShB so it wasn't a "everyone and their video game playing dog" was new type situation.
