I tanked expert roulette on my alts as warrior, 110 crafted weapon, 90 left side, 110 right side. Holding hate wasn't an issue unless I screwed up or someone else did. Runs were 30 minutes max to below 20 minutes depending on the rest of the party.

From a WHM perspective.
1) Either keep pulling or stop. Don't stop to OP or Flash 2-3 times and then keep going. You're taking more damage, I think you're stopping and going to Regen you and start Holying. Flash while running through a group of mobs and then use Shield Lob/Tomahawk to get aggro on ones not hit by flash. This will also keep DPS from ripping mobs off you as they'll start attacking when you stop as well.
2) Use your cooldowns. I have gotten kicked from a run for telling a PLD in AK HM that he needs to actually pop buffs when he does the whole hallway + room for the 2nd trash pull. If I have to blow Divine Seal, Presence of Mind, Swiftcast + Cure II, and Bene and the tank still dies after rotating through those, the tank isn't popping enough buffs. I can't heal 3k+ of unmitigated incoming damage when I only have 2.6k Cure II's.
3) Wait for Stoneskin. I will try to Stoneskin you when the last mob is at 25% or I'll Swiftcast + Stoneskin II after everything dies. But sometimes you run around a corner and I have to start casting all over or Stoneskin doesn't stick on everyone and I have to cast it manually. I don't want to pull aggro off the things you're about to pull, you don't want chase them. So if you see that party arrow on your screen from my casting, just stand still for a second or two and it'll be okay.
4) Don't pull what you can't handle. I know people like speed runs, but if you can't keep hate on a big group or you're just barely on ilevel requirement for the run. Then stick to smaller groups that you're comfortable with handling.
5) Watch a video before the fight. Not as big of a deal for a dungeon, but a tank is supposed to lead the group. So even if it's your first time, watching a video will help you understand what to expect so you don't blindly lead your party into danger or get surprised by a boss mechanic.
6) If you're in a 24 man raid, for the love of everyone's sanity, don't fight the other tanks for aggro (I mean, sometimes you accidentally rip the boss off another tank, but I mean doing it intentionally). I was in a WoD last night and I saw a tank on Cerb's butt provoke after the tail AoE came out, and Cerb cleaved right away. That tank literally 1-shot all 10 melees in that raid and a couple of healers.
7) Compensate for your party. If you're running 2 melee, try to have all of the mobs facing the same way. If you've got 2 BLMs (and you feel comfortable doing so), pull big so they can utilize their OP AoE rotations. If you're doing a low level roulette and 2/4 people are new, ask the 4th person if they wouldn't mind doing a full clear and help the lowbies. :)
8) It's okay to make mistakes. Everyone does. Sometimes we wipe and/or people die and that's okay. Just learn from the experience and try not to take it out on everyone and I promise it'll be okay.

I hit stoneskin and let you develop aggro..by the time it drops off you should have aggro built and I can happily heal to my hearts content,You run thru at high speed pulling everything then screech to a halt and I have to immediately start slamming heals because you couldn't pause long enough to let me apply buffs ...

This is mainly for new tanks (dungeons under level 50).
1) Healing generates aggro. This is very basic, but someone who has not played this type of game before may not know. When encountering a group, a tank may think that they may only need to flash/overpower once, then focus on one enemy if the group has no AoE. But after a few heals, the tank may lose aggro to the healer. You should do your AoE enmity skill twice or tab and do your enmity single target combo on each enemy before focusing on one. Of course, do not overdo it and spam overpower. This is bad.
2) You can wear Disciple of War gear, but try to wear as many "tank" pieces you can find, the ones that say GLA MRD PLD WAR. Wearing the correct pieces of armor plus Vitality accessories will make your healer's life easier.
3) Pulling multiple groups is fine, but ask the healer first. Believe it or not, some healers DO want the dungeon to go faster! Just ask the healer if they are comfortable with it.
If a tank is doing anything that annoys me, it's usually this. I find it almost as bad when I'm playing BLM as when I'm on SCH or WHM since I have to try to slam the brakes on my AoE rotation as soon as I realize that the tank is pulling completely fresh mobs; more often than not I have to throw up my wards, AM to the tank, and hope that it can regain control quickly enough for me to resume my job.
I can deal with having to compensate for tanks who are gearing up, but when they have the gear yet are just doing it wrong, it's a bit more frustrating.
I would ask that tanks try to avoid having tunnel vision.
Having good awareness of the monsters, their actions, and your party are important to both tanks and healers. Paying attention to status effects and the environment are also pluses.
When I tank and stop after a pull, I often swing the camera around so I can see what is going on behind me. That way I can spot any stray monsters and pull them back quickly.
I also keep an eye on the party list, constantly watching the healer's MP as well as HP of all party members. If I see someone's HP going down, it tells me I am missing something so I try to find and correct it.
Also make sure all the mobs are finished off before running to the next patch, and keep an eye on the whole party to gauge if people are ready for the next pull.
There is a lot going on and it's hard to keep an eye on everything at times... but I think good awareness is an important skill for tanks to have.
Not every tank is like this, but I've run into more than a few that just seem afraid to keep all the mobs on them. It makes everything so much more difficult. Mobs start going after everyone else, we all wipe, and I get blamed for not keeping everyone alive. I've also had tanks whoo just keep pulling more and more mobs without a break, or just run into a boss before my MP regenerates. Of course I end up running out of MP, they die, and everyone gets frustrated and the mood starts to get bad.
I try to keep X-Ethers and other MP restoring items, but I can't always rely on them due to the cool time. Just make sure everyone is ready to keep going, especially at a boss, it actually speeds things up and makes the run much more enjoyable.
As someone who mains a pld and in the process of leveling up a warrior I've been guilty of assuming I have enough aggro on the mobs we're fighting only to have one peel away. I'm finding it easier to manage aggro with the warrior compared to the paladin. My most recent blunder with this was in snowcloak with a random group who were all probably higher ilvl than me from the look of their weapons and gear. I know the next time I run that or anything while my gear is where its at I'll need to get more flashes mixed into the rotation.
It's difficult to manage aggro when people outstrip you by a large ilvl margin - I've murdered myself plenty in Garuda EX when the OT is much lower ilvled than I am as healer (and then the sisters just come over to give me all their love).
As long as you're trying to keep the aggro, no one can point blame at you. ^^ I've dabbed a bit into tanking (but never made it to 50.... I find I just do not have the confident enough personality to tank/lead the way) - I feel your pain. DFed Copperbell mines NM as a dinky little gladiator - ending up with poetics geared BLM and NIN for DPS was a horrible battle for aggro during the entire run... lol. I'm only lucky that they only attacked the ones I had targeted, and not attack everything other. *Shudders*

I can understand that people get distracted by real life, I have failed a lot of times due to my husband asking me a question and me trying to answer, but I've had tanks just.... pull and then not do anything, as if they are alt tabbing or something.... Or 3-4 mobs on myself (if I'm healing) or the healer. I usually run by a dps and they start killing them...
Also, cool downs ..... cooldowns, cooldowns, cooldowns!!! Hallowed Ground and Holmgang aren't just for "OH SHIT" moments, use them during pulls, the best thing is the world is seeing the icon and being able to Holy, if WHM, or get DoT's + Bane and Shadow Flare, if SCH, on the mobs. ALSO PLD's ... if you use Hallowed -_- Don't use anything else .... They don't stack, your just wasting everything else after you pop that... if your low, maybe CONV to help the healer, but otherwise Hallowed + Sentinel is .... just wrong ><!
