It seems to be a problem with the level3 hops. But since its a problem beyond the 5th hop, verizon seems unwilling/able to do anything about it, and have adopted a "it will work itself out/its level3/square's problem" stance.
Since its an issue thats been happening (for me) since 2.1 my only guess is that the increases server position check frequency is not jiving with the Level3 switch. This would also seem to explain why im mostly seeing it in dungeons.

It would be nice if SE could tell us what they are doing about this. As we can see by all of the different ISP's listed there is nothing that we as a consumer can do.
SE you are making bucket loads of money off of us. Make some phone calls and get this fixed please....

Issue first noticed 13th/14th January.
Problem occurs about 2 or 3 times every minute in all areas, all movement freezes for 1 or 2 seconds and then catches up, some fates impossible to play, not attempted dungeons, and all activity compromised to one degree or another. All of my other mmo's play without any problems.
ISP: Sky
Country: UK
Timing: Always since around 14th January
Severe latency at all times and makes game content impossible to complete. The problem is very similar to those posted before me with the same ISP. The game has been running fine up until the date specified. Internet connection has remained stable when playing other games / staying online - the only issue is with FFXIV.
ISP: Sky
Country: UK
Timing: Always since 14th January
Occurs in all areas whether cities or outside zones. Movement freezes for a few seconds, actions do not 'go off', rubber banding all the time.
ISP: Sky
Country: UK
Timing: Since the 13th
Frequent occurrences of everything freezing and then shooting everything off in a couple of seconds. Freezes can last up to 30 seconds. This did not happen before the 13th January. Happens in towns. fields and instances.

Lag 24/7 since Tuesday the 14th, completely unplayable (though I am still trying haha)
To everyone on Sky that are experiencing the issue: Get in touch with Sky AND Square Enix. The problem is occurring somewhere between the two, and the best way to get it solved is for both of them to acknowledge that there is a problem. Once they've had multiple people throwing the exact same issues at them, they'll hopefully do something about it.
Square has been told, multiple times (at least twice from me), they aren't motivated to do anything about it.

That is why I think this thread is a waste of time. I haven't see any GM response about issues. I am seeing people from Canada saying about lag and they aren't doing anything. I am going to change my modem and router today and if the lag continue I am not going to continue playing this game......
I sent a ticket and had a live chat with SE support about this. Linked them this thread or another one of the many. Got an e-mail from them saying that they've "escalated" the issue and are looking into how to resolve it, if that puts some ease on anyone's mind.
Guess I'll add myself to the list:

Lag spikes happening all day everyday since 13th Jan
ISP: AT&T UVerse

Lag all over the place all the time always (since 2.1 started).
No reference number from ISP.
Discussed this issue multiple times with SE NA and AT&T. Neither admit fault. I have no problems with other games/internet things. Once this month is over without resolution I'm done.
ISP: Deutsche Telekom
Country: Germany
Occuring ever since the night from January 14th to January 15th, europe time. The night before was fine.
Rubberbanding and lag of up to 5 seconds. I used to have slight lag before 2.1, almost none after 2.1, and now it's close to unplayable.
Increasingly occurs in 8-man instances, not quite so bad in low-level content or the open world.
Have not raised the issue with my ISP.
And to think my subscription just renewed, too... so much wasted time and money.

Previous to the latest update I had no issues. Ping to Montreal was amazing and I didn't experience lag. Now I am constantly dealing with those darn 90000, 2002, 3102 errors again. The last 3 days have been the worst I've personally experienced. Doesn't matter the activity (crafting, talking to NPCs, participating in fates or off chocobo-riding for fun) happens in all zones at all times throughout the day. Very frustrating trying to play. I'm lucky if I can stay connected longer than a half an hour! Can't join duty or dungeons out of fear of dc'ing and forcing my party to boot me. Contacted ISP and tried all methods of suggested troubleshooting. Nothing has helped.
Still no response on this?? Step it up SE, my free trial to WTFast expires in 10 days, and without that proxy server the game is unplayable.

Heavy/Constant Outages at 17:00+ until early morning.
Symptoms are complete modem connection drop and automatic modem cycle.
Results in a range of 90000 errors.
No connection issues otherwise. FFXIV related only.
ISP refuses to acknowledge any event occurring. Refused to open a ticket. Went so far as to claim that the modem never cycles.
Even after asking them If that is the case then why why does it give a new IP and DNS route each time.
Call ended with canned response.
Not much I can do. They have a monopoly for internet in the area.
