Yes, it has been reported.
In game (the only stated acceptable way), via the forums, and even over avenues such as Twitter, we try and report to raise developer, moderation team and player awareness.
Clearly, the methods they use to detect it, or the means by which they can accept evidence are insufficient.
This is why I believe it's important to let other players know that they should report when encountering this type of disruptive behavior, as it's specifically against the Terms of Service, and why I believe that empowering players to submit this type of evidence to the moderation team, and for them to be more open with the playerbase with what their outcomes of their investigations are is just as important.
I struggle to think of a competitive mode that will not have some avenue of manipulation. If they're able to come up with one, more power to them, but as it is I don't think it's unreasonable to suspect that the same players will be doing the same things on future modes. And if only the same methods of addressing them exist, well... How will things change?
At the very least, being able to make other players aware of the disruptive players seems like the best we are able to do at this time.
