Must Love Pigs - Beat Ifrit while all of your party members have a baby Behemoth pet out.
Some really great ideas in here but if I could suggest.. lets not have any achievements that rely on dying in an event. I'd prefer not to see a party chat involve some of the group wanting to fail on purpose and the other half frustrated that they just got half way through to watch a lemming orgy :P.
Also.. we should have some chocobo achievements :D (raising, racing, digging, fighting with)

you never know? Since enemies can most likely still attack you... if a lvl 50 botanist got attacked by a lvl 2 enemy (I know, it can't happen) it wouldn't make much sense for the lvl 50 botanist to eventually become KO'd, although this is all speculation... and if a lvl 50 character did get KO'd by a lvl 1 or 2 monster maybe they should get a trophy then...
Achievement: Wiped at boss 10 times, 100 times, 1000 times
We don't need no stinkin' Limit Breaks: Defeat "boss" without the use of Limit Breaks.
Bone Breaker: Have every part of "boss" incapacitated at the same time.

What are you doing?! You're feeding the mob TP!: Get the kill shot as BLM with a melee attack.

I'd like to see ones that encourage helping others like:
A Classy Fellow/Lady: Help players with level 50 Job quest battles 10 times.
Stick With Me kid: Help a player complete dungeon you have done before.
Charity Is Its Own Achievement: Pass an item in a dungeon chest to another player
Let Me Tell You About The Time I Saved Eorzea: Help players with main storyline quests you have completed already 10 times.
Seems like a general theme is helping other people for achievements. Things you have already completed.
Seems like a good incentive to help others after you have already done something.

Passing on the Legacy - Help a new players with story mode,class quest or dungeons more then once Must be tagged Legacy or use a chara imported from 1.0 or compete main story plus one full class quest line on any job
There is already Chocobo masks in the game, I think getting masks based on each of the mob types would be a nice thing to add.
They could be rewards for getting kill-based achievements, such as killing 10,000 ahriman or whatever.
How is a new player gonna get this achievement if they aren't legacy? There would have to be an equilivent achievement for non-legacy members.
Learning from the best: Complete a dungeon with at least one Legacy Player
No use crying over Onions: Donate a full suit of armor (level 10 or below) to a new player (first class under 10).
I have trained you well: Your personal chocobo defeats 10/50/100/500 enemies (Gets the kill shot).
Deja Vu: Defeat a storyline mission 10 times.
Bestiary: Complete a full crafting/gathering/battle log.
War machine- Master blacksmith and Armorer classes
Ifrit bleeds, we can't kill it: Party gets KO'd by eruptions/plumes during Ifrit fight.
My Turn!: Get the killing blow on a Notorious Monster after being KO'd and raised during the same fight.
This sounds an awful lot like speed runs. There's a reason SE had to nerf the toughest NMs in XI: people played for too long in one sitting and started complaining of health problems. That being said, Primal fights in 1.0 were relatively short. As long as there's time for a break, it'd be okay.

As a second suggestion: Allow the new fame system in GC's allow the player to earn a toggleable standard aka guidon aka banner or flag that they can carry, which differs based on level of fame.
