Raid replacement frames.
I want them so bad. Like, I can't even put into words how badly I want to replace those ugly party frames with something more aesthetically pleasing.
Also: Weak Auras.

yeah. boobs and dongs are basically 99% of what comes out of modding.
No. Weak Auras is an addon in WoW that allows you to display ability cooldowns anywhere on your screen. So instead of staring at my bars to see when I get procs, I could put them where I'm more likely to see them. I'd love to be able to track things like Overcure procs, the CDs on some abilities (like Divine Seal and Benediction), and such, more easily. It also lets you see debuffs on yourself more easily - you can set up a Weak Aura to show a big icon of the debuff when you have it on you. I don't know about you, but my debuffs are tiiiiiny. -_-
The addon looks like this:

The little squares with like, the angel image and the fountains of light are from Weak Auras.
The removal of all gear dye restrictions. Turning the "name become Fresh Meat" and "disable displaying vanity gear" pvp systems into toggles instead of mandatory.
On a grander scale new questlines. The Skyrim modding community made quite a few great homebrewed questlines.

I kinda do something similar, but obviously less robust, already. I have some hotbars set up that I don't intend to ever use to actually trigger an action, but I have my cooldowns set up on them so I can keep track of them easier.
But I also play using a controller, so I don't know how viable this is for a mouse/keyboard person.
