Too small for my hands, I had one for a while lol. It cramped terribly after a bit. Using a naga trinity at the moment, with the 6 button layout.

Perhaps you didn't mean your first statement too literally, but I personally would get hindered more by using an MMO mouse because 1. the button placement looks awkward, no way I'm relying only on my thumb and 2. given where I live the fancier the mouse, the more easily they seem to break. My Polish friend has a Logitech mouse with 2 extra buttons and has gotten use out of it for 5+ years; when I bought the same thing the right click became unresponsive within weeks. Honestly the cheapo wireless mice I use right now with your standard 2+middle buttons has been more reliable.
That's the one I used. I upgraded to Naga Pro after Trinity started double clicking (all mouse brands have gone cheap) and Pro lacks that side panel. Instead they added some bad Battle Royale panel.
I use the 12 button panel but only around 6 of those. Fingers are too big to easily use the buttons in the middle area
An alternative to this MMO mouse, and my preferred one, is the Logitech g600. It's extremely affordable, usually on sale for $40 or less and offers a third mouse click. This allows you to bind that extra click to a modifier key (ctrl, alt, Shift), giving you essentially 24 grid buttons. Basically, you can program almost your entire kit on the mouse itself with this setup.
While it does take a little adjustment, you'd be surprised how quickly using your thumb to press most of your buttons becomes muscle memory. Obviously, this is entirely subjective but I've known several people who worried about the same thing you were and wound up preferring the mouse I linked.
I'm gonna be honest, I thought it would be clunky to use too. But the logitech g600 is a godsend in that their buttons are made so you don't even have to look at where your thumb is, and for most people it's a pretty comfortable grip. It takes a bit to get used to, like most peripherals, but it took me maybe two weeks and now I can't imagine playing without it. My left hand only has to press ctrl or shift, instead of the number keys at the top, and my right does all my movement, and all my button presses, without causing me any pain ahah.
No such thing as button bloat in this game. It's designed to work on Playstation with a controller. If you add macros and find you run out of room, you are running the wrong macros.
I personally play on the PC, I use a "MMO Mouse" and "Razer Tartarus Pro". I Have 4 hot bars set up for my combat Key binds, the hotbars are mostly empty.

--I also have a sidebar that has no keybinds, this is used for start of instance abilities, ie Dance Partner, Tank Stance, etc. ((I also have Living Dead here as it's sadly a never used ability as it always fails to work as indented. Looks at the White mage in the party.
Additionally on my Controller keys 11 to 19 are my F-Keys, which are keybound to my Macro Controlled Hotbar. The Side Hotbar with Macros controls my F-Key hotbar, allowing for changing Jobs, activating other useful tools etc, and then 8 seconds later the Hotbar goes back to default which is seen in the screenshot. In that same corner, I also have hotbars for fun stuff and other uses. You'll See my Bojza Macros, My Custom Changes, and fun emotes. And if for some reason I'm tracking collected items ie Eureka, I'll plop those things on the hotbar to keep track.
You will notice though, as I was in dancer for the alliance raid, that my combat hotbars were mostly empty.

((BTW2 My title is also Ironic, but only noticeable when I'm not in an instance, as my FC on that Character is Lilac Foxes, our FC Tag is <<FOXES>> so the title lines up Lilacs over Foxes, and since that character is the FC owner it's fitting she is the Keeper of Lilacs.))
I just play with a cheap plain keyboard and mouse, there's no need to waste your money on fancy gadgets
