Had someone leave because they claimed their pool was on fire.
Worst I've heard - ended up in a Toto-Rak run on one of my healers when joining leveling roulette, and noticed that the tank was taking an awful lot of damage for that level...
Inspected...half their gear was glamour gear. Actual, honest to Hydaelyn level 1 glamour gear. They had a couple pieces closer to their actual level (which was in the 40s). The right side was missing a piece, and also included a gathering piece and a caster piece.
I decided I'd just focus on the left side issue and asked what happened to their gear.
"Oops, I forgot. I don't have it with me."

He had a 70 dps class, as I recall, so I'm reasonably certain he knew how the armory inventory works.
Also known as "Oops, i left all my low level gear with my retainer and this class is not high enough level to wear the high level stuff i do have with me, and i forgot to retrieve the stuff before queing, i dun goofed"
I have encountered a few people in low level dungeons who used that.
The best: Brb, goldfish on fire! (They were trying to say 'stove is' but said it so fast we all heard goldfish, never let them live it down.)
The worst: It's your fault we are wiping. (A tank to a healer when Shinryu was new after the tank failed the tidal wave mechanic and the healer called him out in it.)
Ran the 73 Dungeon and my co-dps got the stack marker on the first boss ran away from everybody.. we even chased them down. they kept running and died... then they got it again and did the exact same thing. when asked if they know what to do they said there dog needed attention or something like that lol. But it only happened during the stack markers.
Back in ARR, my raid group was fighting Titan for someone's relic. I played our WHM.
I just had surgery on my back and had three locations with 3-4 stitches each. Well, as you know when you start to heal and with any stiches, they get super itchy. I tried to resist, I really did but the itchiness was too much to bear so I took my hands off the M/K to scratch my right shoulder blade. Such a relief, but to my horror I see the wiggly lines of a landslide, right under my character.
"OH !$#&." I yelled into my mic as my character fell to his death.
"My itches were stichy!!! I'm so sorry!" My raid group starting laughing with me and ended up launching themselves off the platform.
Good times.

It's default excuses for all.

Running a Heaven on High party to floor 100, Playing Red mage, tank is taking serious damage from mobs, Whm is dps'ing. Tank dies, whm gets mobbed, I stood there and watched. I asked the Whm why they didn't heal..

my friends and i joined the main scenario instance in roulette and there was this one guy who kept afking. Im not a fan of carrying someone who doesnt even try to help so i called him out. Welp i regretted my decision fast. He told us that he just found out that his cat is pregnant and it was about to give birth (?). Im not sure if he was serious but after 5minutes or 10 he stopped afking and continued playing as if nothing happened xD
Was running First Coil and one of the guys in the group goes "BRB the base is under a mortar attack." We knew he was in the service but he never really told us where he was stationed at the time.
Back in 11 out LS is getting ready to start our runs for the night and the one healer goes "BRB have to poop" he did not come back for 3 days. He was logged in the whole time at the entrance to what we were going to do. His brother who was in our group had no idea where he went.
