I think it's a communication issue. If the tank comes in with the expectation to do big pulls and burn boss while a dps wants a more casual run through of the dungeon doing 1 set of mobs at a time then that is when we get the toxic attitudes

Good healers have way more control over most dungeon runs than any tank, lol. I don't need a tank to kill most bosses, while tanks are lucky if they can kill one without a SMN using physick on them. So I'm still waiting for the day that a tank will dictate rules to me.
The Healer of the run is definitely the one who has the most control over the dungeon. Drama queen tanks only act the way they do because hardly anyone plays tanks. I would rather have a Summoner with Titan out than deal with someone like that.
Healers can easily let people die if they really really wanted. Although in these instances when we all just want to get things done and over with probably not the best time.
.....I may have done that once or twice out of spite in more 'safer' dungeons, however when I don't I do sliiiiiiightly judge them for their behavior. :D
I'm pretty sure the boss isn't attacking when it's trying to raise his moogle friends so the Tank really has no excuse to not help if the moogles are scattered. I've found popping a swiftcast holy and assize help clear the moogles personally unless they are stuck against an awkward side just try to lure all new moogle spawns to you and try to pile them together. The only people who shouldn't be dealing with those moogles though is the DPS any time not spent attacking the boss is a waste.
I often let people die in extremely easy dungeons if the damage they took was fully avoidable/not due to poor tank aggro. It's important to let them fully realize the consequences of their actions (or lack thereof).
If it's a sprout, though, I won't do that. Newbies need time to learn.
True, I've just had too many experiences where people refuse to treat each others like decent human beings in this game and hold people hostage in bosses where a tank is necessary, all because of something incredibly trivial like accidentally letting someone die.
Im not sure if its elitist, but more just lazy/impatient people that are not willing to put in any more effort than their ideal time frame
Its much like a little kid syndrome, 'my way or nothing whaaa'.
And then as you get older, you gradually learn patience, effort, and compromising. (at least hopefully they do)
who wants to spend the full 90 min in expert? I sure as hell dont, of course ill only be vocal if it seems like someone was perfectly doing things then it seems like they handed the keyboard to their 4 year old child during the last boss or something
