It happened something similar to me for about 2 months. I'm legacy, but I started ARR as Arcanist, so I customized my hotbars sizing (msp?) down my pet hotbar and placing it where hotbar #3 use to be for main job (BLM). I ran CM countless times as BLM telling people I didn't know what was going on with cannons refusing to work. One day, suddenly I found the issue: my hotbar #3 was actually covering the new hotbar with the cannon shot... I moved hotbar #3 and there it was the new button... It had been there all the time. A glorious day, indeed :D
I got to level 50 before I realized two things
The colour to the left of the mobs tracks aggro (as a healer it's really usefull)
Focus target (again, helps me so much, for instance in AK, I had to ask people to tell me when the last boss did the big AoE)
So far, the only thing that made me facepalm at my own stupidity was not realizing you can press Shift+M to bring up the mark window. I used to think the only way to get to it was through the menu...
It took me a good while to realize tomahawk was ranged. I was in a party and said "I wish MRDs had a ranged attack like the shield lob." One of my party members said "What about tomahawk?" <facepalm> >.<
So... during our t7 clear (SPOILERS) where healer has to stone an add (SPOILER END), none of our healer knew that you can heal someone facing away from that person. GG
Not really a facepalm moment, as there's some debate whether this is a good idea or not. I've mained WAR since beta. My cross-class skills have always been Flash, Provoke, Convalescence, Featherfoot, and Internal Release (occasionally switched out for Awareness). On a whim, I switched out Internal Release for Haymaker, thinking that I might get some use out of it when there's actually something hitting me. Wow, what a lifesaver. It doesn't work on everything, but cutting a mob's dps in half for 12 seconds is amazing.
I never knew that there was a /busy option. It blocks all tells and I don't get bothered by constant RMT Tells. /facepalm.:confused:
I started testing ARR in Phase 2. I didn't learn how to Shout until Phase 3.
I didn't learn how to use the auto-translate feature until they added in 2.2 patch a quest that teaches you how to use it.
Yep, same thing for me with the auto-translate as well.
I was several levels into the game when I realized there was a "Repair All" option with menders. Prior to that realization I had always repaired my armor one piece at a time. >_<
LOL, I did this for ages. I thought you could only teleport between the shards in the city via using another shard. So if I had to go somewhere in town, I'd always find the closest shard and then tele. orz. It was only when I was doing a bunch of running around with my FC that someone mentioned it to me.
Despite being a FFXI and version 1.0 player, as well as an Alpha and Beta tester, I didn't realize til recently I could simply reply to someone who had sent me a tell via the < r > command - I was forever typing people's names manually when chatting to them in tell. When I discovered it, I felt really stupid. :P
Took me three months to realize that the money you make from selling items through your retainers stay with them. Kept asking myself "Why on earth do they give us the option to bank money with the retainer?" - I thought the money went immediately into your pockets. Nice surprise, when I realized I was wrong.
Took me another month after that to realize I could zoom out the camera....
I didn't know how to limitbreak until about a week ago. As a WHM I never had to use it and when it was needed someone else, like a bard, did it. I finally got to use it just recently. I was so excited. xD
I didn't learn I could mark mobs til my PLD was 45.
...I didn't learn Provoke didn't generate a chunk of hate at the start of the fight til like 47. .___.
I knew you could ferry from Limsa to Vesper Bay but I never knew where the ferry was until today (thought it was the one at Fisherman's Bottoms, it's actually west of Arcanist's Guild).
Playing since launch, I learned only a month ago how to use pet in Obey/Guard mode and no longer on Sic. Makes healing with fairy soooo much easier...
A few months in I learned from a friend that if I looked beyond the blur of icons on the party list, there secretly lie numbers to tell you how far down the threat list you are.
I was level 50 for a month running around with Garuda and kept seeing people saying to use Enkindle, realised I had to unlock it through a quest that I had missed/ignored
I didn't know you could sort a part list until a week ago.
It would have made healing so much easier for Titan fights, lol!
(As a result of not knowing, I got to improve greatly the disposition of my HUD though.)

I didn't know about the mouse over placeholder for macro ("<mo>") until very recently.
This thing is such a charm to use in combination with middle mouse clicks!
I traded ilvl90 items for seals not realising that I was equipped as a CUL and had to grind for 3 weeks to get them back
This thread is great! I didn't realise for quite a while that duty finder was cross server. Thought it was odd that so many people logged straight out of the game after a dungeon, as they were not appearing on my player search
CTRL + U , I learned that one last week. So useful.
I've done some of these.
Took me until AK to know how to focus target, I thought people meant just like target them to see what they were casting lol.
Took me until Haukke Manor HM to realise that Shroud of Saints was handy for regening MP if need be.
Took me until I was level 40 or so to realise that you could teleport. No seriously. I completely forgot about it. I attuned to all the crystals too so I had no excuse to forget lol, I just thought they were for home points. I kept getting the airship from city to city and either choco porting or mounting up myself and running places. It did save me a lot of gil though I must say.
I've also healed with Cleric stance on many times, though that is just by mistake after switching to dps and forgetting to take it off. :P
I didn't realise you could click the Aetheryte in the plaza of the capital cities to access the aethernet until about a month ago. I'd been teleporting to cities, and then legging it to the nearest aethernet shard since launch.

I never new about the boat to Vespar Bay from Limsa Lominsa until well after I reached level 50.
My friend was leveling Blacksmith and was always complaining about how much Bomb Ash he needed. When I got my Miner up to 36 and saw Bomb Ash on my list, I immediately farmed up two stacks and put it all (including the HQ ones) in the FC chest for him. When I told him there were two stacks he asked me how I got it all? Confused, I said I just got Miner to 36 and mined it. He facepalmed and said that he'd been farming ash by killing bombs for hours on end. He had no idea it could be mined.
Took me to level 43 on PLD to use flash correctly. I assumed that it did nothing if used multiple times in a row since it said "blind - immune". Someone was nice enough to explain that it still did enmity generation ugh....
Big crystal in capital cities, yeah. Always ran to the littles up until two weeks ago (been playing since beta).
Only learned how to shoot cannons two weeks ago as well. Never knew to hit the fire symbol AND THEN the "x" button to make it actually fire. I'd decided they only shot for DPS (makes senses, right?) until I saw a healer do it. *shame*
And same with focus target - didn't know until about two months ago it was actually something, different than cycling through targets.
