While it does say use of alcohol and tobacco it is still a teen rating so 13+ having a /smoke emote or a /drunk emote would absolutely require the rating to be upgraded to Mature 17+ possibly adult as it is illegal to serve alcohol in any establishment to anyone under the age of 18 (in UK at least) There is no way SE is going to allow minors to make their playable characters drunk or smoke you cannot have these emotes till the rating has been increased which can only happen with an expansion's launch. Shadowbringers has yet to have an ESRB so depending on that your /smoke and /drunk emote ideas may be possible
And it's entirely up to those parents if they want to let their kids play mature games, but there are also plenty of parents out there who would have their kids' accounts cancelled over a re-evaluation in rating. It's still something that SE would have to weigh the pros against the cons and make a calculated decision on whether to go in that direction.
That being said, there are other games that have such emotes, and a 'drunk' emote would be fun to play around with. As is, however, the closest we have to one is the 'charmed' emote you get from the ananta beast tribe quests which is more comical than anything.
Likely would be far more costly to implement than would be worth it in the long run. Same reason they haven't given us 1.x-era servers or level-cap 75 ffxi servers, which are things that the playerbase of both these games have asked for before. More than likely, whatever they give us will be added in an update to all servers at once, exactly as they've been doing for years.
I really do not know if adding these emotes will change the rating, but if it will, then I don't think creating 18+ only worlds would make a difference because as soon as it's part of the game (and not entirely player created), then it will affect the regulation for the entire game. Also, in the Q&A session in Paris, they mentioned they couldn't even show a particular scene of someone being shot by an arrow even though this game already has violence and death.
