I have almost 5000 in-game achievement points so I have put in some work. I get told all the time that my "of the Golden Leaf" title is awesome because of it's rarity, and how well it works with my character name ;) .
I had my server's first Blue Chocobo (meh), and my server's first Fenrir mount (fame!!, no cactpot win!!).
I have successfully 4man Turn 5, Turn 9, Titan EX, and 7man all of Final Coil.
I actually met someone in the game I felt comfortable enough with to Eternally Bond with (most surprising to me. totally unexpected but totally :D about it also)
I have accomplished every goal I have set for myself except for my last Job to 50 (WAR@48) and BiS Botanist to match my Miner (5 HQ logs away from Botany Lucis)
I should have those things done by the weekend. I am eagerly anticipating Heavensward and can't wait to see what the future holds for me, and am ready to make many more memories with the people I cherish for the entirety of 3.0!!

Honestly, most of what I'm proud of in this game are things that I've accomplished with my static. I've been in the same static since T6 (well, technically T5, but I didn't clear that until well after SCoB was released); only two other people have been in it as long as I have, and one is the static leader. We were way behind the curve in SCoB but we caught up to it during FCoB. Now we're practicing 7 manning T13, and hit the enrage at 5% a couple of nights ago. We're still quite active as a static, and have actually ramped up activity during the content drought to keep us active and doing things together. We run learning parties on Saturday nights to help people with content they haven't been able to learn/clear yet. Other nights we do sells or farming or basically whatever our little hearts desire that keep us doing things together. We don't always get along, but I honestly don't know where I'd be in the game if I hadn't met these people. They are a huge part of my time in FFXIV now and I don't know where I'd be without them. It's something I don't care to dwell on too much.
As for me personally - I have all of my classes to level 50. I've finished four Zeta relics. I've made over a hundred million gil. I've got most of the Gambler set from the Gold Saucer. I've gone all Pokemon on the primal ponies and finally got my Kirin a few days ago. I have both tank mounts. I'm at 2,100/3,000 for the Parade Chocobo. I've got a rank 20 Chocobo (may the mighty Leviabeetus live forever). I've got all my crafters at 3 star or better and have the materials together to get all of my remaining Lucis tools (I've just been lazy to sit down and craft that crap, LOL).

Cleared T13 last night, finally
Currently have cleared T5 45 times and T9 21 times. When I cleared T5 the first time, I wasn't aware that I had fully trained up 6/8 of the group I cleared with. Being thanked for that was a very humbling moment.
I can look around Mor Dhona and see many people whom I was there for many of their firsts. From CT to T9 and upward. It's a good feeling.
Besides those, I'm happy I was able to save up (with help from winning an FC contest) to buy my own small house. The Villa de Valeria isn't much, but it's home to me :)

Getting 100 million gil before hitting level 50 and getting all crafters to 3 star before level 30.

Turning a group of people who could barely get past the adds into a full clear party for Shiva EX.
Doing the same for pretty much every primal except Titan, really.

Tank Twintania and a set of t10 adds as a Monk. Last man standing in titan df with a bunch of bads, dodged mountain buster using featherfoot, took the cleave and auto attack, survived with 17 hp and landed the killing blow.

Forging the lasting friendships I have in, and out, of my FC. Even before hearing the voices behind the faces, you form bonds with people simply over the internet, until you realize how much their friendship has changed your life. Lasting friends who stay with you through the tough times, FC mergers and breaks, but never leave your side, even when everyone else did. Maybe it's not a personal accomplishment in the outright, but it's still an accomplishment I'm so proud of.

I'd say back in 2.0 when Titan HM was crushing the majority of the player base and everyone was struggling, my party dropped him 3 times in 30 minutes using 6 Warriors, 1 Scholar, and a Monk. It seemed like a total cakewalk. Then shortly after that, killed Hydra with 3 Paladin and 5 Warriors and no healers at all. This was when max ilvl was 90 and the average gear was 70's, no echo. Fun times.

- Becoming a Final Witness through sheer effort and never giving up
- Leveling every Class to 50
- Getting 3 star on all, while 4 Star on 5 Crafters
- Pushing through All the Extreme Primals+1-12 of Coils with Core Static Members ( Some people you have to cycle through, but you feel really awesome with those few individuals that stick it out through the hard times with you )
- Zeta on my Main (MNK4LYFE)
- Getting a House Early on. Looking at the Housing situation now, I can definitely be Proud of this lol
- Making it to 3.0 with Awesome friends who intends to go further. You have some who dropped out of the Game for whatever reason, and some you may have had a fallen out with that you dont play with anymore.
- Leading a Static. Many people find themselves unable to do this for whatever reasons, and Doing so over a long period of time is bothersome, but in many ways rewarding. Originally wasn't comfortable with doing so, but to obtain something I wanted, I had to step outside of my Comfort Zone...and Im proud I did.
