I see multiple possible solutions to this matter. Have blacklisting be account-based and not server based, and players who are on one another blacklists are not paired. While questionable in implementation, a cool-down period for queuing (something small, like five to ten minutes?) Could be implemented to players who are kicked from instances, to better enable a replacement of a different player into duty. Or simply disabling someone from rejoining the same instance they have been kicked from (though this may require some more in-depth coding, but will probably be optimal if it is done.)
