I've had this happen on my PS3 (minus the headaches), but usually only when trying to render large amounts of grass/leaves, i.e. fighting treants is always a pain/slowdown.
Your PS4 might be overheating or it's GPU might be going bad.
Try to use some compressed air on it to get some of the dust out. I am not sure what else to suggest, but that symphom you describe, typically points at GPU issues or overheating. Both which could be caused by dust.
Try changing your resolution in the settings. Some TV's have issues with how FFXIV does their resolution (As Far as I know its the only console game that lets you alter the resolution for Pretty vs Economical.)
I had flickering issues on my PS3 with FFXIV. I got a decent HDMI cable and bam, fixed.
Thanks everyone going to try the steps people recommended and if they fail I guess call Sony for Warranty support. No reason for a GPU to begin to fail so early in a Console's lifecycle.
Has your PS4's fan been noticeably loud for long periods of time?
I been having this problem as well, playing the Ps3 version on a Playstation 3D display. Chat log would refresh and reset and it would flicker in certain places in the zones.
Knock on wood I have been lucky. My game play has been smooth
