Would it be possible for you to return it and get a refund? Downloading it off PSN should solve all your problems. If you buy it straight off PSN, as I did, there's no code to register.

Step 3 might be a bit different since they integrated the account services into the Mog Station rather than the old Square Enix account services.
It's not the obvious one on the red-black-white paper. What you need is the code on the back of the little two-page booklet. It should be in Sony's XXXX-XXXX-XXXX format and needs to be registered on the Sony Entertainment Network (through an "Add Codes" option accessible in the PlayStation Store), not in the Mog Station. (Mog Station is used for PC registrations and will be used to add service time to any account, whether PC or PS3, but is not used to initially register the PS3 version.)
