Except I get the exact same symptoms out in the general game world, and not just instances. Constant hiccups, people stopping and then jerking forward a few feet, monsters acting unresponsive and then using multiple moves at once. It's only way more apparent in Instances since you're starring at four specific people, and have longer fights to observe it.
The game was pretty much unplayable (rubberbanding, long delays between actions) for me and my roommate for the last few days (LA - Timewarner). Ended up having to get a subscription to Overplay to make the game playable yesterday.
No, SE has noted they don't violate the TOS. I recommend Pingzapper to be honest. Battleping was very unprofessional like to me. It's still silly we have to pay $4-5 extra bucks a month for stability though.

So the only solution people can offer are these third-party services that basically rid of these lag spikes. I'm not desperate enough to play this game to pay for an additional service, but I do hope that 2.1 promises us a major network fix, if not just some sort of recoding so we don't get these fast-forwarded situations.
Also, the problem is recurring even in open world, menu, actions, npcs, etc. This is not client based as I've tested it with other computers and at different locations. It's entirely SE's network or some faulty provision by Time Warner.
Most cable companies throttle and generally mess up any traffic that even remotely looks like bit torrent.
